191521 Job Description Serves as liaison between the patient and facility/physician. Ensures a continuum of quality patient care throughout hospitalization and oversees provisions for patient's discharge. Assesses, plans, oversees and evaluates the appropriateness of care throughout admission and hospitalization of the patient. Job Responsibility Facilitates patient management throughout hospitalization. Performs concurrent utilization management using evidence based medical necessity criteria. Submits data to management regarding case management and/or quality initiatives. Serves as an in-patient liaison - planning, assessing, implementing and evaluating patient in collaboration with the health care team. Conducts chart review for appropriateness of admission and continued length of stay; Facilitates throughput. Conducts clinical reviews and interacts with third party payers to obtain approval of hospital days, pre-certification and post-discharge eligibility in relation to clinical course. Ensures compliance with current state, federal, and third party payer regulations. Documents the case management process in the medical record; completes and documents a care coordination assessment on the patient. Completes applicable areas of the patients discharge instruction sheet and the patient transfer sheet; ensures case management documentation is current and accurate. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date. Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed. Emergency Department experience, preferred. Duties Description Do you have nursing experience working in a community setting? Rockland Psychiatric Center is recruiting a Registered Nurse 2 Community to work with individuals with mental illness to prepare them for discharge to a skilled nursing facility. Candidates from outside or within State Service can apply. As a Registered Nurse 2 (Community), you would directly provide, oversee, and coordinate nursing and health related care services for a caseload of individuals in a community setting, such as a Family Care Program, Mobile Integration Team, and outpatient clinic; perform clinical case management activities; serve as a member of an interdisciplinary treatment team; and provide nursing-specific clinical, consultative, and instructional assistance to individuals and their families, other disciplines, and health care providers. Minimum Qualifications Candidates from outside and within State Service may be considered for hire under the Hiring for Emergency Limited Placement - Statewide (NY HELPS) program as a Registered Nurse 2 Community if they have: Possession of a license and current registration as a registered professional nurse in New York State; and one year of post-licensure clinical nursing experience providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting. OR Candidates from within State service may be considered for transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.
