RN - Admissions Case Coordinator Position Summary The Admission Case Manager is an active member of the Patient Care Management and ED team. The role combines every function of case management, from Access through Discharge planning, and post-discharge from the ED. The case manager serves, manages, and supports internal and external customers, communicates appropriately using good interpersonal skills, participates in performance improvement initiatives and programs, participates as a team member, and is accountable for own work responsibilities. RN - Admissions Case Coordinator Position Requirements Education Bachelor’s degree preferred. Required License and/or Certification Current RN license in the state in which you reside. Experience Minimum of 1 to 3 years experience in an acute care hospital. Home health, case management experience with an insurance carrier or HMO and/or Utilization Review and discharge planning experience preferred. RN - Admissions Case Coordinator Schedule 12 hour shifts with 24/7 coverage. Assigned shift may require rotation to opposite shift.
