Assists patients in becoming ambulatory with appropriate equipment and in a safe manner. Removes splints or prostheses in transferring the patient from one location to another. Encourages patients to help themselves whenever possible. Provides the patient with good body alignment and positioning when in wheelchairs or on walkers. Follows designated patient care plans in helping patients with active range of motion (AROM) and passive range of motion (PROM) as assigned. Monitors and reports daily activities of patients to include evaluating and documenting patient progress. About Total Life Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Kansas City area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow. Responsibilities Utilizes the nursing process to develop and implement a comprehensive, age-appropriate plan of care for stable and critically ill patients and families which promotes optimal patient outcomes. Meets the educational and informational needs of the stable and critically ill patient and family, utilizing appropriate principles of teaching/learning. Promotes safe, efficient, family-centered care to stable and critically ill patients. Delegates nursing care based on knowledge of staff qualifications/competency and patient care requirements. Participates in performance improvement activities and attends education programs, unit meetings and assigned committees. Qualifications Associate's Degree ADN Required. Current employees hired before September 1, 2013 are grandfathered and permitted to continue in the direct care nursing role. 2 years or more of nursing experience or Bachelor's Degree Preferred for all nurses hired after September 1, 2013. Nurses hired after this date without a BSN, must complete their degree within 5 years of their hire date. The pay range for this job begins at $35.90/hr, but your offer will be determined based on your education and experience. Remote Work/Work from Home This position is not eligible to work remotely, which means that the person hired will be required to work onsite at one of our Children’s Mercy locations and may not work from home.
