Home Therapy position PURPOSE AND SCOPE: The professional registered nurse Home Therapies RN CAP 2 may be an entry level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from RN CAP 1. This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations. Purpose of Your Job Position The primary purpose of your position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs/GNAs and other nursing personnel. Participate in Center surveys or inspections made by authorized government agencies, as may be requested. Periodically review the resident’s written discharge plan. Participate in the updating of the resident’s written discharge plan, as required. Assist in planning the nursing services portion of the resident’s discharge plan, as necessary. Interpret the department’s policies and procedures to personnel, residents, visitors, and government agencies, as required. Admit, transfer, and discharge residents, as required. Complete accident/incident reports, as necessary. Provide information to the Billing Office as necessary to insure accurate resident billing. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
