Develop, implement, and evaluate individualized health care plans for students with special or chronic health needs. Maintain accurate health records; coordinate referrals, immunizations, communicable disease control, and compliance with NJDOE and Board regulations. Provide health counseling and education to students, families, and staff; serve as a resource on health-related matters. Collaborate with child study teams, administrators, and medical personnel to support student wellness and participate in district health program development.
