Job Description Summary: Job Description: ​ Job Responsibilities (listed in order of importance and/or time spent) Competent in the clinical management of home infusion patients including Patient management skills, the Nursing Process, the rules of Federal, state, and local regulatory entities that oversee and control the provision of care in a Home Health Agency. Knowledgeable of documentation and billing requirements for home health/ infusion care goods and services. Determines suitability of patients for home care. Supervises, monitors, and evaluates the quality and effectiveness of patient care, including, when appropriate, in home evaluations of field staff. Reviews and monitors appropriateness of Nursing Care Plans and directs the staff to modify the plan as needed. Acts as Administrator and/or Director of Nursing of the Home Health Agency for Medicare certified home health agencies in the absence of a director or Manager of Nursing. Manages daily activities with regards to effective operations and communications and ensures the office remains in compliance with all regulating and accreditation entities. Complies with reporting requirements, ensures policy and procedure, compliance and monitors and evaluates medical supply usage. Participates in the process for assessing, managing, and reporting nursing operational and financial data to the Regional Nurse Manager, Area Director of Nursing, and/or Director of Operations. Supports the development and implementation of the Plan of Care for each patient. Develops, implements, evaluates, and participates in office specific continuing education and orientation programs. Assists the Director of Operations, Area Director of Nursing and/or the Nurse Manager in the recruitment, development, and monitoring of new staff. Coordinates and participates in on-call responsibilities. Maintains confidentiality of patient and proprietary information. Participates as a member of or directs the professional advisory committee, infection prevention and process improvement teams, including conducting chart audits. Basic level skill in Microsoft Word (for example: opening a document, cutting, pasting, and aligning text, selecting font type and size, changing margins and column width, sorting, inserting bullets, pictures and dates, using find and replace, undo, spell check, track changes, review pane and/or print functions). Oversees the collection, compilation, and analysis of clinical program activity data to evaluate clinical performance and competence. 10. Provides technical assistance, resource services, consultation and education to clinical specialty service relative to orientation, ongoing assessment of competency and performance improvement activities. 11. Serves as a clinical role model and resource to the nursing staff. 12. Prepares reports and analysis associated with orientation of new hires, ongoing competency and evaluation of staff performance and identification of related outcome measures. 13. Provides timely status reports of results of assignments including identification of patterns and trends that require actions. 14. Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations. 15. Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits. 16. Participates in daily activities that reflect budgetary constraints and incorporates financial principles into program development and allocation of resources. 17. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies. 18. Participates in the performance appraisal process of designated Nursing staff. Qualifications Master's Degree in Nursing is required. Masters in Nursing or health related field is preferred. NP Certification in specialty area preferred. 5 years directly related experience (some supervisory preferred) New York State License with current registration as a registered professional nurse. Certification in specialty area preferred.
