Contacts Physician for verification of prognosis and appropriateness for Hospice/Palliative care. Contacts and interface with team members, primary physician, team physician, administrator, and other staff to ensure patient’s and family’s needs are met. • Works directly with outside vendors and care delivery partners, to ensure when patients are transferred to General Inpatient Facilities, Emergency Departments, and Paramedicine Activation, that documents and patient information to ensure continuity of care and most effective care based on patient/caregiver goals and clinical needs. • Provides clinical directions to patients/caregivers in accordance with Hospice guidelines. Obtains and receives verbal orders from team/physician and documents care using standard form. Evaluates and documents effectiveness of directions through telephone follow-up with patients/caregivers. Description Patient Care Associate MSH GP9C Med/Surg FT Nights EOW As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Works collaboratively with the attending physician, consulting physician(s), and other disciplines to identify, develop, and implement an appropriate plan of care that maximizes individual patient/ family preference and enhances quality, access, and cost-effective outcomes. Prescribes in-patient and discharge medications according to New York State law, including controlled substances with DEA authorization. Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol. Practices as member of the allied health staff according to the rules and regulations of the medical staff and bylaws as outlined in the delineation of privileges. Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols. Considers needs and behaviors of specific patient age and cultural groups in all patient care. Promotes own professional growth and development in clinical and managerial role. Maintains current expertise in area of practice. Serves as a clinical expert and resource for the education of peers and other health professionals. Participates in the development and monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvement. Uses evidence-based healthcare literature to advise and support appropriate practice changes within the designated service. Works collaboratively with nursing and other disciplines in the development and implementation of clinical studies within the area of expertise. Assists in the preparation, implementation, and evaluation of research protocols when applicable. Monitors and trends the cost effectiveness of the position, particularly as it relates to the efficiency and performance indicators established for service.
