Position Overview The Registered Nurse (RN) Supervisor plays a key leadership role in managing nursing operations during assigned shifts at Constant Care Agency’s partner nursing facilities in the Bronx. The RN Supervisor ensures that residents receive safe, high-quality care consistent with the standards of practice, regulatory requirements, and the mission of Constant Care Agency. This position requires strong clinical judgment, supervisory ability, and compassion for the elderly and long-term care population. Primary Responsibilities Clinical Supervision Oversee all nursing and direct care staff (RNs, LPNs, CNAs) on assigned units or shifts. Ensure timely and accurate delivery of nursing care in accordance with physicians’ orders, facility policies, and resident care plans. Monitor residents’ conditions; respond promptly to changes in status and initiate appropriate interventions or physician notifications. Conduct walking rounds at the beginning and end of each shift; verify continuity of care and accurate reporting. Oversee medication administration and treatments to ensure safety and compliance. Staff Leadership & Management Assign, direct, and evaluate the performance of nursing staff; delegate tasks appropriately based on scope of practice. Provide mentorship, education, and on-the-job training for new or less experienced staff. Ensure adequate staffing levels; manage call-ins and replace staff as needed in collaboration with the staffing coordinator. Promote teamwork, professionalism, and accountability across the care team. Regulatory Compliance Ensure nursing practices comply with New York State DOH , CMS , and facility policies . Conduct chart audits to confirm accurate, complete documentation of resident care and progress notes. Participate in QA/QI programs, infection control audits, and safety committees. Prepare for and participate in DOH inspections and surveys. Maintain resident confidentiality and HIPAA compliance at all times. Emergency Response Serve as the on-site clinical authority during assigned shifts. Respond to medical emergencies, perform immediate assessments, and direct staff response. Communicate critical incidents to the DON or Administrator on call. Resident & Family Relations Foster compassionate, resident-centered care and ensure residents’ dignity and rights are upheld. Communicate effectively with residents and families regarding care issues or changes in condition. Participate in care plan meetings when required. Qualifications Licensure: Current, unencumbered Registered Nurse (RN) license in New York State . Experience: Minimum 2–3 years of RN experience , with at least 1 year in a long-term care or skilled nursing facility . Prior supervisory experience strongly preferred. Education: Graduate of an accredited nursing program (BSN preferred). Certifications: BLS required; ACLS preferred. Knowledge: Comprehensive understanding of NYS DOH and CMS nursing home regulations. Familiarity with MDS documentation, infection control, fall prevention, and pressure injury protocols. Skills: Strong clinical assessment and problem-solving ability. Excellent leadership, delegation, and interpersonal communication skills. Ability to handle emergencies calmly and professionally. Proficiency in electronic health records (e.g., PointClickCare, SigmaCare). Work Conditions Work performed in a skilled nursing facility environment. Requires standing and walking for extended periods and occasional lifting up to 25 lbs. May require evening, night, weekend, or holiday shifts. IMT participants interact with homeless services, criminal justice, and behavioral health service systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served. IMT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, criminal justice, peer support and behavioral health recovery. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant's choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week. Overview The Registered Nurse (RN) functions as a primary practitioner who assesses and monitors IMT participants' clinical health and response to treatment. The nursing approach of assessment, planning, intervention, implementation and evaluation is used to assists individuals in their achievement of mental health, rehabilitation and recovery goals. Core Principles The job responsibilities of all staff extend to understanding and incorporating certain principles into their work and into their relationships with program participants. Clear professional boundaries to support the limits and possibilities of services. Essential Job Functions Develop, implement and maintain IMT program nursing standards. Guide the development of policies and procedures and identification of resources essential to delivery of nursing services. Conduct psychiatric, physical health and other assessments with a special focus on conducting assessments and evaluations with individuals using substances or experiencing street homelessness. Develop, or consult and coordinate with team to develop, plans to address participant health needs. Monitor and modify treatment plans as indicated by participant responses and conditions. Use online databases and portals e.g. PSYCKES, Healthix, MAPP, and others to support assessment and treatment planning. Order, interpret and evaluate medical diagnostic tests to identify and assess participant conditions. Provide management and administration of medication in conjunction with the psychiatrist. Provide a range of treatment, rehabilitation, and support services. Utilize motivational interviewing techniques and a trauma informed approach when delivering individual and group based health services. Administer IMs and vaccines, plants and reads PPD's, and performs phlebotomy as per orders Monitor vital signs and side effects of medications and reports findings to team, and within annual health assessments. Collaborate with psychiatrist and other medical personnel to schedule appointments and coordinate care. Make referrals to community physicians and work jointly with hospital and institutional staff. Intervene and advocate on participant's behalf with, for example, collaterals, agencies, facilities and family members; assist with problem solving to help participant access needed supports. Provide consultation to IMT team regarding participant medical issues; serves as a resource to the team on medical concerns. Maintains accurate, detailed reports and records including required documentation, progress notes and health related data, in AWARDS. Evaluate and establish system for reporting on nursing intervention and expected outcomes. Attend and participate in supervision, meetings and training sessions as required. Provide 24-hour crisis intervention services on rotating basis. Job Qualifications Commitment to person-centered treatment strategies, upholding participants' rights, and self-determination in service provision. Currently licensed as a registered professional nurse by the New York State Education Department. Minimum two (2) years' experience working as an RN. Minimum two (2) years' experience working with persons with mental health condition in health or social services setting. Excellent written, verbal and interpersonal communication skills.
