Advanced Practice Provider (APP) – Gynecology Location: Fox Chase Cancer Center (Philadelphia, PA) and satellite locations Schedule: Full-Time | Monday-Friday Position Summary Fox Chase Cancer Center is seeking an Advanced Practice Provider (PA-C or CRNP) to join our Gynecology team. This role is ideal for someone interested in delivering high-quality, patient-centered care to women with a broad range of benign gynecologic conditions as well as gynecologic cancer survivors. The APP will play an integral role in providing comprehensive outpatient evaluation, treatment, survivorship care, and care coordination while collaborating closely with gynecologic surgeons, gynecologic oncology providers, and a multidisciplinary care team. The APP will work alongside dedicated physicians and fellow APPs in both shared and independent clinic settings. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. I. Qualifications Skilled Nurse from an Accredited program currently licensed and in good standing with the Pennsylvania State Board of Nursing. One to 2 years’ experience in nursing in an acute care setting or 1-year experience in home health, hospice or comparable experience is preferred. III. Orientation and TRAining Tender Care Hospice will provide orientation and training, as necessary, to assist the Case Manager Nurse in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file. IV. On Call The Case Manager Nurse will be on call according to scheduling. During the hours when on call, the Visit Nurse is expected to follow On Call Protocol and to respond to all pages within 20 minutes. V. Accountability Reports directly to the Nurse Case Manager and Director of Nursing. The Nurse Case Manager will work under the direction of the Director of Nursing.
