Works with this team to identify and track key performance measures including but not limited to patient satisfaction, discharge disposition, and specific outcomes, as necessary. Assists in the development and revision of standards of care, policies, clinical pathways, and order-sets that pertain to the care of patients. Responsible for collecting and recording data pertaining to certification standards. Routinely presents this data to a target audience and engages this audience in achieving benchmark targets. Monitors ongoing documentation of care by healthcare providers for patient populations. Ensures compliance with regulatory and quality standards such as those established by IDPH, the Joint Commission, and other regulatory agencies. Utilizes various methods to increase program-specific awareness to staff. Coordinates with educators in the development of education programs for staff orientation, annual education, and competencies. Works with nursing, rehabilitation, and other services to ensure comprehensive patient education and preparation is achieved and maintained, on an ongoing basis. Works with interdisciplinary leadership to ensure patient education materials are standardized and consistent with order- sets and clinical pathways. Works with appropriate staff to review and approve materials, as necessary. Participates in staff education on an individual and/or group basis. Provides support to the team for continual learning. Works with quality and performance improvement departments to distribute reports to key stakeholders. JOB SUMMARY Assisting the Director of Nursing Services in the planning, organizing developing and directing of the day-to-day functions of the Nursing Department in accordance with current federal, state, and local standards, guidelines and regulations, and facility policies. Operating as a CNA providing direct care to residents, the objective is to ensure the highest degree of quality care is maintained at all times. QUALIFICATIONS Must have current certification as a CNA Must possess current CPR Certificate or become certified within two weeks of employment. Demonstrate interest and/or knowledge in the therapy, rehabilitation, restorative, ADL, splint, and ROM program. Must be able to read, write, and speak the English language in an understandable manner. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to deal tactfully with customers, family members, visitors, personnel, government agencies/personnel, and the general public. Must possess leadership and supervisory ability and willingness to work harmoniously with personnel. Must be personable, enthusiastic, and resourceful. Provides feedback to clinical leadership regarding CNA daily and annual performance evaluation feedback. Make daily rounds to ensure that nursing personnel are performing required duties, and to ensure appropriate procedures are being followed. Make physical rounds of all customers daily. Make rounds with physicians as necessary. Maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department. Plan, conduct and participate in In-service Education Programs. Place orders for equipment and supplies as necessary following established budgetary guidelines and assists with the distribution of supplies to ensure that the CNAs have all necessary equipment and supplies. Assist in developing methods for coordinating nursing services with other customer services to ensure the continuity of the customers total care regimen. Monitor and perform administrative duties such as completing forms, reports, studies, charting, etc., as necessary. Monitor absenteeism to ensure that an adequate number of nursing care personnel are on duty at all times. Report problem areas to the Director. Assist and maintain general organization of nursing units. Provide the Director of Nursing with information relative to the nursing needs of the customers and the nursing service department’s ability to meet these needs. Assist with arranging appointments and transportation as needed. Assist support services in developing, implementing, and conducting in-service training programs that relate to the nursing services department. Ensure that all personnel involved in providing care to the customer is aware of the care plan and that care plans are used in administering daily care to the customer. Attend Department head meetings, Infection Control meetings, Quality Improvement meetings, etc., as directed.
