As the Afterhours Registered Nurse's your primary focus is to conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours, including at night and on the weekends. This is a full-time salaried Hospice RN position providing home visits for hospice care. Requirements Active RN license in North Carolina required Prior hospice and/or home health experience required Must be able to travel throughout the agency's 50-mile service area On-Call Schedule Weekday on-call: Monday through Friday, 4:30 PM to 8:00 AM Weekend on-call: Friday 4:30 PM through Monday 8:00 AM Schedule rotates one week on, one week off Position includes scheduled visits and admissions as patient needs warrant Primary Responsibilities: Able to triage patient needs over the phone until a nurse can provide an in-person visit Responds appropriately and with urgency to crisis calls from patients and families afterhours Makes the initial nursing evaluation in determining eligibility for hospice services, as part of a patient visit, within forty-eight (48) hours of referral if assigned Competent to perform afterhours visits including admission visits, death visits, recertification visits, and routine visits Identifies the patient/family's physical, psychosocial, emotional, and environmental needs and reassess them as needed, no less than every fifteen (15) days Documents problems, appropriate goals, interventions, and patient/family response to hospice care Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. The Nurse Practitioner/Physician Assistant will perform physical exams, collect and document data, conduct diagnostic and therapeutic procedures, order and schedule laboratory studies and sub-specialty referrals, prescribe appropriate interventions and medications, and provide overall care coordination services. The PNP/PA will contribute to excellence in patient care, research, teaching, and provide leadership in the program. The Pediatric Nurse Practitioner (PNP)/Physician Assistant (PA) may also perform inpatient consults on infants from the pediatric wards who have discharge planning needs and will be following up in SICC or PCNC, such as those needing be discharged home with NG tubes. In this role, the Nurse Practitioner/Physician Assistant performs a thorough chart review, physical exam, and provides recommendations to the inpatient team to support the infant’s feeding or development and management related to their transition from hospital to home. Family Nurse Practitioner (FNP) or Physician Assistant (PA) Prior APP Experience Required APP Experience in Family Medicine or Urgent Care Strongly Preferred Key Responsibilities: • Assessment of Health Status • Performs and documents in the medical record a complete history and physical examination for acute and complex chronically ill patients • Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. • The nurse practitioner may serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions. • Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. • Coordinates daily clinic schedule with the attending physician team, presents patient report, collaborates in plan of care, and follows assigned patient population. • Diagnosis • Manages diagnostic tests through ordering, interpretation, performance, and supervision. • Formulates differential diagnoses by priority. • Diagnoses complications and orders appropriate interventions. • Formulates Plan of Care • Identifies expected outcomes from diagnosis and formulates and documents a plan of care to address complex acute and chronic health care needs. • Utilizes evidenced based practice guidelines or protocols in an individualized, dynamic plan of care that can be applied across the continuum. • Implements and modifies plan of care • Prescribes diagnostic strategies and therapeutic interventions both pharmacologic, non-pharmacologic, and surgical, needed to achieve expected outcomes. • Manages further diagnostic tests through ordering, interpretation, performance, and supervision. • May perform advanced procedures consistent with privileges and competency validation, including minor suturing, complex wound management, line and tube placements and removals • Communication and Collaboration • Maintains ongoing communication and collaboration with interdisciplinary health care team. • Facilitates and communicates with patient, family and staff to promote continuity of care across the outpatient continuum of care. • Documentation • Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and/or clinic note where applicable. (h&p, daily progress notes, plan of care, problem lists, procedure notes, acute event note, discharge summaries in medical record per specific patient unit or service, death summary) • Documentation is timely, meets critical care compliance standards and captures patient acuity. • Professional Practice • Demonstrates Professional Practice behaviors including: preceptor/mentoring, education and instruction of students, nursing staff, nurses, graduate and novice nurse practitioners, • Seeks opportunities for active engagement in research and the analysis of evidenced based practice. • Actively participates in Grand Rounds, APN council meetings, faculty and unit meetings, M&M presentations. • Maintains CEUs, and membership in a professional organization.
