Job Description Job Function Summary: Involves the provision of integrated and accessible primary and specialty patient care services in a clinic or center environment. Care is provided by clinicians and assistants who are accountable for addressing a large majority of personal health care needs and developing a sustainable partnership with patients. Ensures optimal patient outcomes through effective leadership and / or provision of patient care delivery in healthcare clinics or centers. Promotes shared decision-making at all levels to ensure a shared voice in practices, exchange of ideas, enhanced communication, and continually improved patient services. Management levels may also be responsible for clinic operations. (Note, when primary focus is clinical operations, see Ambulatory Administration job function.) This Nursing Leadership role oversees pediatric patient care and workflows for a variety of surgical specialties programs including: dermatology, general surgery, gastroenterology, urology, plastic surgery, ophthalmology, OHNS. The Nurse leader manages the delivery of ambulatory patient care, which typically includes nursing, and may include laboratory, radiology, dental, etc. Facilitates the flow of patient care activities; may also participate in practice or center management operations. They are part of an outpatient nursing management team that supports pediatric patients and their families for both clinic visits, care coordination and procedures throughout BCH . Department Overview UCSF Benioff Children’s Hospitals (BCH) Ambulatory Services provides approximately 450,000 outpatient clinic visits annually, with activity spanning over 20 unique sites of care across the 9-County Bay Area and beyond, and with clinical services spanning primary care, medical specialties and surgical specialties. BCH Ambulatory Services comprises practices with faculty from many different academic departments across the UCSF School of Medicine including pediatrics, neurology, surgery, orthopedics, otolaryngology, urology, ophthalmology and neurosurgery. To improve access to service in the community and remain competitive in the Bay Area regional marketplace, BCH Ambulatory Services is expanding into both affiliate and satellite locations which will entail different models of activation and clinical oversite. This position plays a leadership role in ensuring alignment with clinical services and workflows so there is consistency across the BCH Ambulatory enterprise. This position is 100% on-site and must be able to travel to multiple locations, such as San Francisco and Oakland. Job Description Under the direction of the Ward 86 Nurse Manager, the Registered Nurse (RN) will work in a primary care setting, delivering comprehensive nursing care within a multi-disciplinary team structure. Responsibilities include, but are not limited to, providing direct patient care, administering medications, coordinating patient care, conducting assessments, and managing sexually transmitted diseases. About Ward 86 Clinic Ward 86, the HIV/AIDS Clinic at the Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG) campus has been a leader and innovator in field of HIV care since the beginning of the epidemic. Ward 86 is the largest clinic providing primary medical care for people living with HIV for the San Francisco Health Network (SFHN) and also provide preventive services to patients at-risk for HIV infection, including pre-exposure prophylaxis (PrEP). Job Description Join our amazing team of Psychiatric Nurse Practitioners at UCSF Health, where you will serve patients directly within the Emergency Department and the inpatient setting! The psychiatric nurse practitioners will be performing evaluations, placing and releasing legal holds, serving as both consultative and direct patient care role. Evaluating the need for inpatient hospitalization and assisting with dispositioning and after care needs. Responsibilities Work with physicians/surgeons/APP colleagues and other clinical and administrative staff to ensure efficient patient management based on accepted practice, including but not limited to, taking histories, performing physical examinations, peri-operative planning/coordinating with outside providers and primary care providers (PCPs) as applicable, responding to changes in patient status, providing prescriptions/evaluations/referrals, diagnostic tests/treatments as permitted under core privileges and standardized procedures. Evaluate for and order home health services, durable medical equipment (DME), Department of Motor Vehicles (DMV) certifications, refer for consultations, physical therapy, order radiographic imaging or diagnostic tests, and specialized treatments (e.g. chemotherapy with the approved privilege). Provide patient and family education, arrange follow-up appointments, and engage in any approved privileges after the APP has demonstrated competency through proctoring and FPPE process. OMAG will identify independent practice for approved privileges on the ECHO website and procedural activity will be entered by the provider and monitored via the eLOG system. The Cardiothoracic Surgery Advanced Health Practitioner position has clinical, procedural, administrative, and educational responsibilities in the care of adult patients requiring cardiac and thoracic surgery. The CT surgery service provides advanced surgical treatments, including coronary artery bypass grafting, aortic and mitral valve replacement, extracorporeal membrane oxygenation, and heart transplantation. Our CT surgery APPs manage patients collaboratively with CT Surgery Fellows and Physicians, perform bedside procedures, and assist in the operating room. The Hospice Team Nurse provides professional nursing care to patients by utilizing all elements of nursing process. 2. . Assesses and evaluates patient’s status 3.
