Perform nursing procedures within the LPN scope of practice, including catheter care, tube feedings, dressing changes, ostomy care, specimen collection, range-of-motion exercises, and other ordered treatments. Provide IV and IV-site care as permitted by state regulations, licensure, training, and facility policy. Communicate effectively with residents, families, physicians, nursing leadership, and other members of the interdisciplinary team. Participate in shift report and resident rounds and ensure important information is communicated between shifts. Help maintain a warm, calm, respectful, and resident-centered environment. Ensure nursing staff follow established safety procedures and infection-control practices. Protect resident privacy and maintain confidentiality of all resident and health information. Report incidents, concerns, or suspected inappropriate disclosure of resident information in accordance with facility policy. Perform other duties as assigned. What We’re Looking For Current, unencumbered Licensed Practical Nurse (LPN) license in the applicable state. Current CPR certification. Ability to communicate clearly and professionally with residents, families, and team members. Strong clinical judgment and the ability to make appropriate decisions within the LPN scope of practice. Ability to provide leadership and support to nursing assistants throughout the shift. Knowledge of nursing practices, documentation requirements, and regulations applicable to senior living or long-term care. Long-term care, skilled nursing, assisted living, or senior living experience is preferred, but not required. A team-oriented attitude with a genuine commitment to providing excellent resident care. Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed. May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary. Key Responsibilities: Coordinate and complete the Minimum Data Set (MDS) assessments in accordance with federal and state regulations.
