Job Summary: The Nurse Practitioner (NP) is responsible for the assessment of selected patients in the designated service. The NP performs physical examinations, confers with patients in person and by phone/electronically, collaborates with others in the department to assure smooth flow of patient care consistent with established quality indicators and participates in the overall coordination of patient care. The NP reports to the Chief of the designated service and to the Department Manager. Reviews medical record for follow up or ordering of diagnostic studies under established protocols. Establishes and maintains communication with other health care providers and departments to ensure operational efficiency and effectiveness and to promote consistency and continuity of care. Establishes effective working relationships with members of the health care team, patients, and families. Acknowledges staff rights and cultural and ethical beliefs. Delegates appropriately and coordinates duties/activities of health care team members. Evaluates effectiveness of care given by team members. Demonstrates knowledge of and applies safety principles as identified by the institution. Performs efficiently in emergency situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Demonstrates responsibility and accountability for own professional practice. Participates regularly in staff development activities for unit and department personnel. Demonstrates knowledge of legal issues, including patient confidentiality and risk management in all aspects of patient care and unit functioning. Participates in unit and department Performance Improvement activities. Participates in nursing research activities as requested. Identifies and solves problems effectively. Demonstrates a service philosophy in all interactions with patients, families, and all members of the health care team. Demonstrates proficiency in the use of computers and telecommunication modalities to document, track and convey information. Essential Job Functions/Responsibilities Responsibilities of the home care aide include, but are not limited to, the following- · Execute the client’s care plan as outlined by the Care Designer. · Help clients with their ADLs (Activities of Daily Living) that are within the scope of practice for a Registered Health Care Aide. · Assisting clients with personal hygiene such as skin, mouth, hair care, bathing and dressing. · Assisting clients in and out of bed, transfers, positioning and assisting with ambulation. · Assisting with prescribed exercises which clients and aides have been taught by appropriate health personnel. · Meal planning and preparation, including therapeutic diets, and assisting clients with eating. · Assisting clients to the bathroom or in using commodes, bedpans or urinals and incontinence care. · Performing household services including housekeeping, laundry, transportation, correspondence, making telephone calls, shopping for personal care items or groceries, and companionship. · Assisting clients with medications reminders which are ordinarily self-administered. · Reporting changes in the client's condition and needs to the administrator or care designer. · Completing records regarding services performed. · Driving to appointments or to run errands. · Accurately records all hours worked. · Remains alert throughout the shift in case client requires assistance, responds immediately to any such needs by providing the assistance required, or calling the Company for guidance. · Participating in all required entry-level and annual training requirements. · Maintaining an active home care aide registration with the State of Ca. Dept. of Social Services Home Care Services Bureau. · Other duties as assigned by the administrator or designee. Position Qualifications · At least 18 years of age. · Current registration with the State of California Home Care Aide Registry. We Provide paid training and help you with the registration process. · Completion of a criminal background check and disclosure of any criminal history or evidence of exemption. · Evidence of entry-level training or plan to complete paid training prior to client care and in accordance with CA regulations. · Evidence of tuberculosis testing and verification that the home care aide does not have active tuberculosis. · Ability to read and follow written instructions and document care given. · Self-directing with the ability to work with little direct supervision. · Empathy for the needs of the ill, injured, frail and the impaired. · Demonstrates tact, patience, and good personal hygiene. · Valid driver’s license and auto insurance and/or reliable transportation required. Physical Demands · Regularly required to sit, use hands or fingers, handle, or feel and talk or hear. · Required to reach with hands and arms. · Required to stand, walk, climb or balance, stoop, kneel, crouch, or crawl, and taste or smell. · Must be able to lift and or move up to 10 pounds, frequently lift and or move from 25 up to 50 pounds. · Specific vision abilities required by the job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. The professional nurse is accountable to the Director of Patient Care Services for implementing the patient plan of care and communicating each patient needs to the Interdisciplinary team. The professional nurse is also responsible for communication and collaboration with community physicians as well as family members regarding individual patient care. The Hospice Team Nurse provides professional nursing care to patients by utilizing all elements of nursing process. 2. . Assesses and evaluates patient’s status 3. Initiates the plan of care and makes necessary revisions as patient status and needs change. 4. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. The Hospice Team Nurse completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. POSITION QUALIFICATIONS 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. 7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
