As a Full-Time Nurse Practitioner you will provide high-quality primary and preventive care to client employees at our health center located in New York, NY, conveniently situated near Central Park. This position offers a consistent Monday through Friday schedule from 8:00 AM to 5:00 PM , allowing you to build meaningful patient relationships while maintaining a healthy work-life balance. What You’ll Do Conduct physical exams and preventive health measures per guidelines Performs procedures such as injections, suturing, wound care, and infection management Develops patient care plans, records progress, and ensures continuity of care Educates patients on treatment plans, health maintenance, and development Orders and interprets diagnostic tests to assess clinical conditions Collaborates with physicians and healthcare teams for comprehensive care Prescribes or recommends treatments, including medications and therapies Refers patients to specialists or physicians as needed Maintains patient health records ensure accurate and up-to-date documentation and records Educates and coaches nursing staff on best practices Performs other duties as assigned What You’ll Bring Maintains state RN licenses/certificates to practice Maintains Nurse Practitioner licenses/certificates to practice Maintains State Prescriptive Authority (if applicable) Federal DEA license (matching each state of practice as applicable) State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration) Current national certification (ANCC or AANP required) Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required) Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting Work-life balance is at the foundation of how decisions are made and where Premise is headed. The RN Care Manager for the Bone Marrow Transplant (BMT) service supports a highly vulnerable population requiring intensive, multidisciplinary coordination across the inpatient and outpatient continuum. In this role, you will drive care coordination, promote efficient patient flow, and ensure safe, patient-centered transitions through proactive collaboration with the interdisciplinary team. Key Responsibilities Serve as a core clinical member of the interdisciplinary team, leading daily interdisciplinary rounds to ensure clear, timely communication of patient care plans, goals, and discharge readiness. Coordinate comprehensive care for BMT patients across the continuum, recognizing the complexity and vulnerabilities of this population. Perform initial and ongoing nursing assessments to evaluate clinical needs, identify risks, and support care planning. Conduct utilization review to ensure patients meet criteria for the appropriate level of care and support optimal use of acute care resources. Proactively identify and resolve barriers to patient flow, collaborating with physicians, nursing, social work, pharmacy, and ancillary services. Develop and implement safe, patient-centered discharge plans, including coordination of post-acute services such as skilled nursing, home infusion (IV therapy), and durable medical equipment (DME).
