Job Description The Medical Coordinator is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $30.00 - $35.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. Schedule is 3pm-11:30pm, Tues-Sat. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge. Link clients to medical services when necessary (insurance, PCP, specialists, referrals). Coordinate home health visits (if needed) and follow-up on medical appointments. Promote health literacy by providing patient education with regards to clients’ illnesses, medications, preventative measures, treatment management and overall general health. Arrange transportation for clients to and from medical appointments, as needed Report emergency situations to Supervisor/Manager and document incident Seek proper level of care to assess and treat medical complication Initiate medical response system when appropriate Support patient recuperation by providing necessary items and space needed to rehabilitate and assisting with the upkeep of the facility.
