RN Supervisor – Evening Shift Silver Stream Rehabilitation and Nursing Center Monday–Friday | 3:00 PM – 11:00 PM | Full-Time Silver Stream Rehabilitation and Nursing Center is seeking an experienced and dedicated RN Supervisor to oversee our evening nursing operations. This leadership role is responsible for supervising nursing staff, ensuring high-quality resident care, and maintaining compliance with all clinical and regulatory standards. Responsibilities: Supervise and support LPNs, CNAs, and other nursing personnel during the evening shift Ensure residents receive quality care in accordance with care plans and physician orders Monitor resident conditions and address clinical concerns as they arise Provide guidance, coaching, and support to nursing staff Communicate effectively with physicians, residents, families, and interdisciplinary team members Investigate and document incidents, accidents, and resident concerns Ensure compliance with facility policies and state and federal regulations Assist with admissions, transfers, discharges, and staffing needs as necessary Maintain accurate clinical documentation and oversee nursing documentation practices Qualifications: Current Registered Nurse (RN) license in the Commonwealth of Pennsylvania Previous supervisory experience in a skilled nursing or long-term care setting preferred Strong clinical assessment and leadership skills Excellent communication and organizational abilities Ability to make sound clinical decisions and effectively manage staff Benefits: Competitive salary Health, dental, and vision insurance Paid time off and holiday pay Opportunities for professional growth and advancement Join a team committed to providing exceptional care in a supportive and resident-focused environment. Our Certified Nursing Assistant – CNA (State Tested Nursing Assistant-STNA in Ohio) work with community management to provide seniors with personalized care, and give resident status updates at the beginning and end of each shift. Nursing assistants check in with residents, assist with dining and personal care needs, and perform vital sign checks and clinical procedures according to community policy. Provide assistance with activities of daily living (showers, bathroom assistance, dressing/grooming) Successful completion of State CNA/STNA course is required. Must maintain certification.
