Measure vital signs, Administer IVs, change dressings, clean wounds as needed Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations. Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload. Responsibilities: Monitor Facility Quality Measures and analyze clinical data metrics to identify areas for improvement Ensure all MDS assessments are completed accurately, timely, and in full compliance with state and federal regulations. Opens MDS assessments, fills in resident's name, assessment reference date, and reason for the assessment. Reviews all validation reports from the State Department of Health and corrects errors as necessary. Transmits MDS data to State DOH after making certain assessments are edited and locked.
