Patient Assistance: Assist patients with dressing, undressing, feeding, and other daily activities as assigned. Vital Signs: Take and record oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure as ordered, ensuring competency in these skills. Skin Care: Monitor and care for patients’ skin to prevent tissue breakdown over bony prominence. Reporting: Observe and report any significant changes in the patient’s condition to the Registered Nurse. Mobility Support: Assist with patient ambulation, exercise, and use of bedpans and urinals as directed by the nursing staff. Environmental Maintenance: Keep patient living areas clean and orderly and assist with light laundry needs. Documentation: Adhere to the organization’s documentation procedures, ensuring accurate and timely record-keeping. Professional Conduct: Maintain high standards of personal and professional conduct, respecting the confidentiality and privacy of patients, caregivers, and team members. Safety: Ensure patient safety by using equipment properly and adhering to safety protocols. Professional Development: Participate in personal and professional growth activities, including attending at least twelve (12) hours of continuing education programs per year. Engage in quality assessment performance improvement initiatives and attend interdisciplinary team (IDT) meetings. Respectful Care: Be respectful of patients' and families’ environments and personal needs, providing care that is sensitive to their preferences. CERTIFIED NURSING ASSISTANT (CNA) – Qualifications/Experience Currently licensed in VA as a Certified Nursing Assistant High School Diploma strongly preferred Self-directed with the ability to work with little direct supervision. Is secure with issues of death/dying. Provides a calm manner when in a patient’s home. Possesses and maintains current CPR certification.
