Field RN- CHHA Located in Westchester County, NY An amazing certified healthcare agency located in Westchester County, NY is seeking experienced Field RN to join their team. Responsibilities of the Field RN: Performs a home care assessment to determine patient’s eligibility for services. If not eligible/appropriate for home care services COC will indicate not admitted in the system and reason for the determination. If appropriate for home care services, the COC will complete a comprehensive assessment utilizing HCHB. In conjunction with patient’s family and physician, develops and implements the Plan of Care based upon a comprehensive physical, psycho-social and environmental assessment. Provides skilled nursing care as described in the 485-Plan of Treatment, such as, but not limited to, wound care, injections, prepour/prefill of medications, disease management, medication management, etc.. Orients and supervises home health aide personnel in accordance with regulatory requirements and documents accordingly. Evaluates the effectiveness of interventions in accordance with the plan of care. Identifies the need for evaluation by other disciplines such as physical therapy, occupational therapy, MSW, speech therapy and nutrition. Observes signs and symptoms and changes in patient’s clinical, psychological and functional status. Consults with physician regarding changes in the treatment plan. Educates, counsels, supervises patients and caregiver relating to disease management and medical regime. Documents in HCHB Clinical Software all assessments, treatments and services provided and patient response to the treatment plan. Case manages respective caseload which includes, but is not limited to, ongoing communication with MD, case conferencing with supervisor, obtaining updates from interdisciplinary team, completion of interim orders, updating of patient profile, conducting recertification assessments, etc. Contacts physician to report, clarify and/or obtain orders for; medication changes/additions, precautions, treatment, changes in visit frequencies, additional services needed, requests for supplies and equipment, plans for discharge from a service or the agency. Position Overview The Registered Nurse (RN) Supervisor plays a key leadership role in managing nursing operations during assigned shifts at Constant Care Agency’s partner nursing facilities in the Bronx. The RN Supervisor ensures that residents receive safe, high-quality care consistent with the standards of practice, regulatory requirements, and the mission of Constant Care Agency. This position requires strong clinical judgment, supervisory ability, and compassion for the elderly and long-term care population. Primary Responsibilities Clinical Supervision Oversee all nursing and direct care staff (RNs, LPNs, CNAs) on assigned units or shifts. Ensure timely and accurate delivery of nursing care in accordance with physicians’ orders, facility policies, and resident care plans. Monitor residents’ conditions; respond promptly to changes in status and initiate appropriate interventions or physician notifications. Conduct walking rounds at the beginning and end of each shift; verify continuity of care and accurate reporting. Oversee medication administration and treatments to ensure safety and compliance. Staff Leadership & Management Assign, direct, and evaluate the performance of nursing staff; delegate tasks appropriately based on scope of practice. Provide mentorship, education, and on-the-job training for new or less experienced staff. Ensure adequate staffing levels; manage call-ins and replace staff as needed in collaboration with the staffing coordinator. Emergency Response Serve as the on-site clinical authority during assigned shifts. Respond to medical emergencies, perform immediate assessments, and direct staff response. Communicate critical incidents to the DON or Administrator on call. Resident & Family Relations Foster compassionate, resident-centered care and ensure residents’ dignity and rights are upheld. Communicate effectively with residents and families regarding care issues or changes in condition. Participate in care plan meetings when required. Qualifications Licensure: Current, unencumbered Registered Nurse (RN) license in New York State . Experience: Minimum 2–3 years of RN experience , with at least 1 year in a long-term care or skilled nursing facility . Prior supervisory experience strongly preferred. Education: Graduate of an accredited nursing program (BSN preferred). Certifications: BLS required; ACLS preferred. Knowledge: Comprehensive understanding of NYS DOH and CMS nursing home regulations. Familiarity with MDS documentation, infection control, fall prevention, and pressure injury protocols. Skills: Strong clinical assessment and problem-solving ability. Excellent leadership, delegation, and interpersonal communication skills. Ability to handle emergencies calmly and professionally. Proficiency in electronic health records (e.g., PointClickCare, SigmaCare). Work Conditions Work performed in a skilled nursing facility environment. Requires standing and walking for extended periods and occasional lifting up to 25 lbs. May require evening, night, weekend, or holiday shifts.
