Position Summary Provide wound, ostomy, and continence (WOC) nursing in the prevention, health maintenance, therapeutic intervention, and rehabilitative and palliative nursing care to persons with select disorders of the gastrointestinal, genitourinary, and integumentary systems. Directs efforts to improve quality of care to individuals of all ages directly through hands on care and indirectly as educators and consultation services. Education Current/valid state of NJ RN license (or legal permission to work from the NJ Board of Nursing). Completion of an accredited Wound Ostomy Continence Nursing (WOCN) full scope specialty education program with subsequent board certification in WOC from Wound, Ostomy, and Continence certification board (WOCNCB). Maintain recertification every (5) five years. Completion of general nursing orientation, CPR certification and unit-based orientation. BLS Required from American Heart Association. Experience Computer and keyboard skills. Medical terminology required. Skills Physical and cognitive requirements: Positions physical requirements necessitate a full range of body motion including but not limited to, physically assisting patients, requiring stretching, bending, lifting, sitting, kneeling, stooping, standing, walking, pushing and pulling. Position is classified according to the USDOL as Very Heavy Work which is defined as exerting in excess of 100 pounds of force occasionally, and/or in excess of 50 pounds of force frequently, and/or in excess of 20 pounds of force constantly to move objects. The National Institute of Safety and Health (NIOSH0 guidelines, which recommends that the average worker not lift more than 51 pounds without assistance, will be followed. Manual and finger dexterity and hand-eye coordination are also required as is constant talking and listening in English. Must demonstrate effective oral and written communication skills, including speech clarity, understanding the information relayed, deductive and inductive reasoning skills (the ability to apply general rules to specific problems to produce answers that make sense and the ability to combine pieces of information to/from general rules or conclusion respectively). Considerable walking, standing, sitting, reaching, filing, typing and photocopying required. Ability to perform work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure. Ability to function competently in stressful situations and changing work environment related to changing needs of patients with acute, chronic and complex and/or terminal disease processes. Ability to work cooperatively within health system, multidisciplinary team members (i.e. MDs, Rehab, Respiratory, DI, Laboratory, etc.). Ability to utilize effective time management skills to set priorities to perform responsibilities, problem solve and deliver care. GENERAL JOB SUMMARY An exempt clinical position where the nurse practitioner (NP) is responsible for providing direct patient care. The NP participates as a leader of the skilled nursing facility (SNF) care team. Visits managed care and fee-for-service patients at skilled and long-term levels of care in designated SNFs facilities. Provides appropriate evidence-based geriatric medicine. Coordinates care with hospitalists, primary care physicians and care managers. Makes home visits as directed by the medical staff to meet patient needs and provide continuity of care. ESSENTIAL JOB FUNCTIONS Maintains privileges in multiple Nursing Homes as directed by ACA Maintains license and malpractice insurance Consults supervising attending as needed Documents patient visits electronically at least 90% of the time Participates in documentation and other quality improvement programs Available via phone weekdays 8am- 7pm and when on call. Will reviews, approves, and modifies admission orders Creates a detailed admit note for each admission within 24 hours of patient admission to SNF, including medication reconciliation Initiates/documents Advanced Directives Determines if Health Care Proxy status is correct and invoke if appropriate On weekends, takes call for admissions and see new patients within 24 hours of admission on a rotating basis with other practitioners in the program. Following meals: HHA cleans tables and surrounding area including floors. residents need to be reminded to wash hands and toilet. HHA makes sure resident clothes are clean and presentable. HHA escorts residents to activities and participates. HHA does laundry as scheduled. HHA straightens up common areas as needed throughout the day. Follows proper procedures in emergency situations and responds promptly and positively to resident requests for assistance - including emergency pull cords, telephone calls and requests from family and friends. Throughout the shift: Incontinent residents are changed regularly and remain clean and dry. HHA is always aware of the whereabouts of their designated residents and notifies their supervisor immediately if unable to locate a resident. Encourages teamwork through cooperative interactions with co-workers. End of shift: HHA signs ADL sheets. HHA gives cross-over report to the following shift. HHA makes sure incontinent residents are clean and dry. Resident rooms are in order and no soiled products left in the garbage HHA signs daily assignment sheets and places in wellness office. Compensation The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $64.8517 - $64.8517 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
