Key Responsibilities · Conduct psychiatric and physical health assessments for program participants. · Develop and coordinate care plans with the multidisciplinary treatment team. · Monitor participant health status and modify treatment plans as needed. · Administer medications, injections, vaccines, and perform phlebotomy as ordered. · Monitor vital signs and medication side effects and communicate findings to the treatment team. · Coordinate medical appointments and collaborate with psychiatrists and community physicians. · Provide treatment, rehabilitation, and recovery support services. · Advocate for participants and assist them in accessing community resources and services. · Maintain accurate clinical documentation, progress notes, and medical records. · Participate in team meetings, supervision, and required trainings. · Provide crisis intervention services on a rotating basis. Qualifications · Currently licensed as a Registered Professional Nurse (RN) by the New York State Education Department. · Minimum two (2) years of experience working as a Registered Nurse. · Minimum two (2) years of experience working with individuals with mental health conditions in a health or social services setting. · Strong written, verbal, and interpersonal communication skills. · Ability to assess situations, synthesize information, and make appropriate clinical decisions. · Eligible for full and unconditional participation in Medicaid and Medicare programs. · Computer proficiency including MS Word, Excel, PowerPoint, and electronic health systems. · Comfort providing services in community settings such as shelters, public spaces, and outreach environments. · Ability to travel within the community, including use of public transportation. · Bilingual Spanish preferred. · Must be fingerprinted and cleared by the New York State Justice Center. Job description: We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support. Your expertise in managed care, patient assessment, and medical terminology will be essential in ensuring the well-being of our patients. Key Responsibilities: Provide direct nursing care and clinical oversight for individuals with developmental disabilities in accordance with OPWDD guidelines. Administer prescribed medications and treatments, monitor for side effects, and maintain accurate medical records. Conduct health assessments and respond to medical emergencies as needed. Collaborate with interdisciplinary teams, including DSPs, medical providers, therapists, and family members. Develop and update individualized nursing care plans and contribute to person-centered service planning (ISP). Educate staff on health-related issues, proper medication administration, infection control, and other medical protocols. Administers medications when appropriate, and supervises AMAP certified staff in medication administration. Supervises all medication prescribed for individuals in accordance with agency policies. Provides supervision and training for Licensed Practical Nurses and/or less experienced. Provides immediate first aid with referral to outside emergency services or private physician. MAJOR FUNCTIONS: Under general direction, the Nurse Practitioner is responsible under direction of a physician for providing general medical care, preventative care and treatment to patients in one of the agency’s clinics. This includes diagnosis, treatment, consultation and follow-up care. This individual attends to the patient’s need with the utmost patient care, compassion and understanding. This work is carried out in support of the mission. DUTIES AND RESPONSIBILITIES: Performs routine physical examinations and preventive health measures within prescribed guidelines and instructions of a physician based on assigned program. Orders, interprets and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs. Records accurate medical histories, physical findings and formulates plan and prognosis based on patient’s condition. Documentation includes medical evaluations, diagnosis, procedures, treatment, prognosis, referrals, education and consultation. Discusses case with physician and other health professionals and therapeutic team to prepare comprehensive patient care plan. Submits health care plan and goals of individual patients for periodic review and evaluation by physician. Prescribes medications and monitor effectiveness, or other forms of treatment such as physical therapy, inhalation therapy or related therapeutic procedures. Refers patients to physician or to specialized health resources for treatment. Makes patients aware of importance of a healthy lifestyle. Educates patients and their families on their preventive care, medical issues and use of prescribed medications and treatments. Analyzes new data gained from conferences, training workshops, professional literature and on the job training, where they will assimilate it into their clinical work. Keeps abreast of changes in field. Performs other duties as requested. REQUIREMENTS: Education, Experience and Skills Required: Master’s degree from an accredited advanced Nurse Practitioner program At least Two (2) years’ experience working as a Nurse Practitioner preferably in addiction medicine or substance use and/or mental health treatment programs Knowledgeable about federal and state regulations governing medical/clinical treatment in substance use treatment programs Ability to demonstrate sensitivity to patients with a variety of psycho social issues License/Certification Required: NYS RN and NP (Adult and Family specialty areas) licensures. Valid DEA registration. eMedNY Provider. Valid NPI. BLS Certification. Preferred Education, License/Certification, Experience and/or Skills: National Board Certification within 12 months of hire.
