Evaluates patients’ status, instructs appropriate staff regarding significant techniques, and acts as a resource person to the health care team. Job Responsibility Reviews all protocols to maintain quality control and patient safety, and to assist Physicians with patient’s compliance. Participates in quality meetings to discuss cases and review clinical outcomes. Meets the educational needs of families and other health care professionals concerning the care of the patients. MDS Coordinator - SNF Located in The Bronx, NY Salary: $125K to $165K range (based on experience) Responsibilities of the MDS Coordinator-Nursing Home: Completes assessments, Minimum Data Set (MDS) and care plans for all residents assigned Monitors completion of MDSs by other disciplines within timeframes prescribed by regulatory guidelines Advises supervisor of incomplete and/or untimely assessments by disciplines other than nursing. Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents. Initiates care plans and supporting activities that will result in best possible outcome for assigned residents. Generates and distributes monthly care plan calendar for the following month. Conducts care plan conferences for assigned residents. Requirements of the MDS Coordinator-Nursing Home: Must have a current New York RN license Must have long-term care experience Must have current experience as an MDS Coordinator Must know MDS 3.0 Have a strong understanding of the MDS process Have strong decision-making and problem-solving skills #1370
