Summary The Stroke Program Nurse Navigator is a registered nurse who reports to the Director of Emergency Services and is responsible for supporting clinical teams throughout the hospital who provide direct care to stroke patients, and their families. Patient navigation refers to the assistance offered to patients and their families to help them move through the complexities of the healthcare system and overcome any barriers to quality care. The Stroke Program Navigator recognizes that every patient and every stroke is unique, and that care must be individualized. The candidate will work closely with the Stroke Program Manager and function as a point person for patients and families during and following their hospitalization.
