We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support. Your expertise in managed care, patient assessment, and medical terminology will be essential in ensuring the well-being of our patients. Key Responsibilities: Provide direct nursing care and clinical oversight for individuals with developmental disabilities in accordance with OPWDD guidelines. Administer prescribed medications and treatments, monitor for side effects, and maintain accurate medical records. Conduct health assessments and respond to medical emergencies as needed. Collaborate with interdisciplinary teams, including DSPs, medical providers, therapists, and family members. Develop and update individualized nursing care plans and contribute to person-centered service planning (ISP). Educate staff on health-related issues, proper medication administration, infection control, and other medical protocols. Administers medications when appropriate, and supervises AMAP certified staff in medication administration. Supervises all medication prescribed for individuals in accordance with agency policies. Provides supervision and training for Licensed Practical Nurses and/or less experienced. Provides immediate first aid with referral to outside emergency services or private physician. Includes specific, objective measurable goals. 3 . Consistently demonstrates facility interdisciplinary approach to patient care. This includes but is not limited to: Integrating findings of other members of the interdisciplinary team in treatment assessment and recommendations. Integrating input from patient and family in treatment assessment and recommendations. Preparing for and providing meaningful contributions to the rehab team conferences, patient care conferences, utilization review meetings, family conferences, and caregiver training sessions. Collaborating with and informing facility staff in areas of patient progress. Educating members of the team in areas of expertise. Working with a facility to teach documentation practices which assist the respiratory therapy team. Communicating effectively with discharge planners. 4. Contributes to professional development of self. · Maintaining current professional status and licensure. · Adhering to professional standards of conduct and clinical code of ethics and practice. · Implementing in-services and training. · Maintaining a current body of knowledge in areas of clinical expertise through seminars, professional journals, and peers. · Meeting continuing education requirements of regulatory agencies. 5. Aids in building and maintaining the facility’s reputation as the unparalleled respiratory therapy provider by contribution as a professional member of the staff. · This includes but is not limited to oversight of the following departmental operations: · Consistently demonstrating the facility’s core values by interacting with co-workers, patients and families, with all other persons involved with the patient’s care. · Consistently demonstrating professional standards as outlined in the facility’s Employee Handbook and as communicated by the facility’s management. · Contributing to a positive work team by sharing information, using problem solving methods and accepting new ideas, constructive criticism, or advice from others. · Maintaining close communications within the facility with the director of nursing, nurse managers, and admissions coordinator to assist in development of the respiratory therapy program. · Establishing and maintaining an effective rapport with the facility administrator, MDS, Rehab Director, DON, and medical director. o Demonstrating superior customer service through courtesy, consistent follow-up and positive communication. 6. Demonstrates safe working practices . This includes but is not limited to: Identifying and correcting safety hazards or notifying the facility or the admin so that safety hazards will be immediately remedied. Maintaining work areas in safe and orderly fashion. Maintaining equipment in safe working order by facilitation annual calibration. Using proper body mechanics during treatments of patients and during support activities. Using universal safety precautions. Wearing appropriate protective equipment. The functions of this position will include the following: Shall be assigned patient caseload by the Patient Care Coordinator or designee Assume primary responsibility for a patient caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process Initiate communication with attending physicians, Hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient and loved ones/caregivers Maintain regular communication with the attending physician concerning patient and loved ones/caregivers Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support Provide accurate and timely documentation in electronic medical records. Participates in and contributes appropriate information for Interdisciplinary Group meetings Accept responsibility for coordinating physical care of the patient by teaching primary caregivers, volunteers, and employed caregivers or by providing direct care as appropriate Meet regularly with the Hospice nursing staff to review problems or unique issues from caseloads; share professional support; and exchange feedback aimed toward enhancing professional growth Attend patient care conferences and facilitate discussion of issues from caseload for full staff discussion, consultation and evaluation Supervise and document the NAC every two weeks or as assigned Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues Assume the role of associate nurse when responding to patient/loved ones/caregivers of other primary nurses during on-call time or as requested by the Patient Care Coordinator to fill patient/family needs during the absence of other primary nurses Provide appropriate support at time of death and period of bereavement Participate in the Hospice orientation and in-service training programs for professional staff Participate in agency and community programs as required to promote professional growth and understanding of hospice care Maintain familiarity with policies of the agency and rules and regulations of State and Federal bodies which govern hospice Accept other assignments as appropriate About You: Possession of a current license to practice as a registered nurse in the State of NJ At least two years of work experience as a professional nurse preferably at least one of the years in hospice care Ability to assess and respond to the needs of patients and loved ones/caregivers in varied settings.
