Responsibilities: Provide high-quality preventive care to residents in skilled nursing facilities Conduct on-site visits and remote work from home, supported by a dedicated scribe team Build strong relationships with facilities and maintain a patient-centered approach Be a collaborative team player, working closely with our clinical directors, regional managers, and supervisors Document patient care efficiently and accurately Make your own schedule and enjoy a flexible work arrangement Requirements: Active NP or PA license (in the state of practice) 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills What We Offer: Hybrid Role!
