GENERAL SUMMARY Responsible for assisting the Clinical Nursing Director in the planning, delivery and management of direct patient care for an assigned unit/ group, utilizing the nursing process and adhering to the standards of nursing practice. Utilizes the nursing process for the delivery and management of patient’s care across the continuum of care appropriate to the developmental age of the patient population. Collaborates with primary and associate nurses and other members of the health care team related to patient plan of care. Mentors the nursing staff in the primary nursing role utilizing the principles of the primary nursing model. Develops and monitors systems that promote and support primary nursing practice. Interacts with staff nurses for the purpose of assessing the care provided and the nurses’ capabilities to provide care in changing circumstances. Provides consultation to patients, family members and nursing staff as issues arise regarding the patient’s hospitalization or care experience. Utilizes the quality assurance and performance improvement process to assess the response to nursing care from patients, families and others. Uses clinical skills to provide ongoing assessment and evaluation of nursing care. Coordinates and directs patient care to ensure patients’ needs are met and hospital policy is followed. JOB DESCRIPTION Job Summary Provides screening, preventive primary care and medical care services to members - primarily in non-clinical settings where members feel most comfortable, including in-home, community and nursing facilities and “pop up” clinics. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Provides general medical care and care coordination to various and/or specific patient member populations – adult, women’s health, pediatric, and geriatric. • Performs comprehensive evaluations including history and physical exams for gaps in care and preventive assessments. • Addresses both chronic and acute primary care complaints, and demonstrates ability to ascertain medical urgency. • Establishes and documents reasonable medical diagnoses. • Seeks specialty consultation as appropriate. • Orders/performs pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptoms; works within an environment of limited resources and therefore uses diagnostic tests judiciously and appropriately. • Understands when a member's needs are beyond their scope of knowledge and when physician oversight is needed. • Creates and implements a medical plan of care. • Schedules appointments for visits when appropriate. • Provides post-discharge coordination to reduce hospital readmission rates and emergency room utilization. • Performs face-to-face in-person visits in a variety of settings including in-home, skilled nursing facilities, and public locations. • Performs face-to-face visits via alternative modalities based on business need, leadership direction and state regulations. • Orders bulk laboratory orders to target specific member populations. • Performs alternating on-call coverage to triage any urgent lab results and pharmacy inquiries and develops appropriate plans of care. • Participates in community-based “pop up clinics” to build relationships with communities, and address gaps in health care. • Drives up to 120 miles a day on a regular basis to a variety of locations within the assigned region. Drives beyond 120 miles as part of extended mileage may be required on special project days. Special projects may include an overnight hotel stay. • Obtains and maintains cross-state license in other states besides home state based on business need. • Collaborates with fellow nurse practitioners to develop best practices to perform work duties efficiently and effectively. • Actively participates in regional meetings. • May prescribe medications and perform procedures as appropriate. • Performs timely medical records documentation in electronic medical record (EMR) computer system. • On occasion, may be required to walk flights of stairs while carrying up to 50 lbs. of equipment. • Engages in practices constituting the practice of medicine in collaboration with and under the medical direction and supervision of a licensed physician to the degree required by state laws. • Local travel required (based upon state/contractual requirements). Required Qualifications • At least 1 year of experience as a nurse practitioner, or equivalent combination of relevant education and experience. • Active and unrestricted national certification from one of the following organizations: American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC). • Current state-issued license to practice as a Family Nurse Practitioner (FNP). License must be active and unrestricted in state of practice. • Prescriber Drug Enforcement Agency (DEA) license with authority to prescribe per state qualifications. License must be active and unrestricted in state of practice. • Current Basic Life Support (BLS) certification. • Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements. • Ability to work within a variety of settings and adjust style as needed - working with diverse populations, various personalities and personal situations. • Ability to work independently with minimal supervision and demonstrate self-motivation. • Responsive in all forms of communication. • Ability to remain calm in high-pressure situations. • Ability to develop and maintain professional relationships. • Excellent time-management and prioritization skills; ability to focus on multiple projects simultaneously and adapt to change. • Excellent problem-solving and critical-thinking skills. • Strong verbal and written communication skills. • Microsoft Office suite/applicable software program(s) proficiency, and electronic medical record (EMR) experience. Qualifications The Medical Assistant (MA) is an integral member of the healthcare team and is accountable for patient care as assigned by a licensed independent provider (LIP: MD, NP, PA), a Licensed Practical Nurse (LPN) or a Registered Nurse (RN). The MA participates in the direct and indirect care of patients, health maintenance, patient instruction, collaborative planning and rehabilitation within scope of practice and consistent with his/her education and training. The Medical Assistant may be present in secure medication storage areas and is allowed incidental access to medications. Developing relationships with patients, families and colleagues, the MA functions as an interdisciplinary team member who provides quality patient care and contributes toward the attainment of important goals for the patient, the practice and the organization. PRINCIPAL DUTIES AND RESPONSIBILITIES: Job duties and responsibilities may vary by department (see addendum A for more detail) Clinical Duties and Responsibilities: - Demonstrates true engagement and enthusiasm to contribute toward the care delivery needs for patients and families. - Promotes an inclusive environment supportive of patient and family centered care for a diverse community. - Accepts assignments for patient care within the scope of his/her preparation and within current standards of practice. - In conjunction with a Licensed Independent Provider (LIP) , LPN or RN, participates in the gathering of data that contributes to the assessment, planning, and evaluation of care delivered. - Collaborates as a member of the health care team, contributing toward a supportive and therapeutic environment that promotes the attainment of patient goals that are specific, measurable, action-oriented, realistic and timely. - Accurately obtains vital signs, elicits information about the patient’s basic health status, administers relevant care within scope of practice and as directed by the LIP/LPN/RN, and documents as appropriate in the electronic health record. - Performs routine tests that may include but is not limited to: electrocardiograms, vision and hearing screening, and the collection of specimens for laboratory testing and point of care. - Participates in team-based care and patient education initiatives to support the health and wellness of patients in conjunction with the LIP/LPN/RN. Reinforces patient instruction as directed by LIP/LPN/RN. - Participates in organizational and unit-based quality improvement initiatives and compliance readiness. - Participates in telephone screening of patients; may provide instruction to patient when directed by the LIP/LPN/RN. - Participates in prescription management tasks including pending of new prescriptions and renewals requiring LIP approval, prior authorizations and medication reconciliation. - Follows approved clinical practice guidelines (CPG’s) and workflows, pends appropriate orders for radiology, laboratory testing and other care needs requiring LIP authorization. - Performs a variety of tasks, including those that may be facilitated by administrative staff, which have an effect on the overall operations of the unit. These include flow, stocking of rooms, scheduling tasks and all other duties as assigned. - Appropriately utilizes organizational resources including management of time, supplies, equipment and other assets. - Other duties as assigned. SKILLS/ ABILITIES/ COMPETENCIES REQUIRED: Technical Skills/Competencies Required: Clinical Skills: - Knowledge of practice operations and standards. - Beginning to intermediate knowledge and use of medical terminology. - Ability to accurately perform vital signs and obtain pertinent health information including but not limited to: temperature, pulse, respirations, blood pressure, height, and weight. - Ability to participate in the medication reconciliation processes and related prescription management tasks, including prior authorizations. - Knowledge and ability to perform specific procedures and tests as appropriate to the clinical setting. Communication Skills: - Demonstrates effective oral and written communication skills in English. Communicates in a professional, courteous, clear and concise manner. Facilitates therapeutic relationships between patients, families and other members of the health care team. Writing Skills: - Ability to correspond and communicate with others clearly and effectively in writing (via e-mail, memo or interoffice note, documentation of patient information) and to take complete and accurate messages. Phone Skills: - Ability to telephonically support the practice by answering and screening calls, offering information within scope of practice to facilitate communication. Organization Skills: - Ability to manage work processes in a neat and systematic way. Ability to manage multiple tasks effectively, appropriately prioritize clinical work, follow established policies, procedures and clinical practice guidelines, workflows. Technology Skills: - Demonstrates ability to utilize technology in the clinical setting. Includes basic computer skills as well as the utilization of electronic medical equipment.. Ability to type accurately and enter data at an intermediate level. Ability to become proficient in using the EHR and other appropriate applications. Ability to utilize hand-held technology and download related data to the electronic health record.
