Responsibilities include: Performing patient assessments, H&Ps, daily progress notes, and ongoing management of hospitalized burn patients. Providing patient and family education, communicating treatment plans, and serving as a key liaison for families throughout the hospitalization. Coordinating care with multiple disciplines, including surgery, nursing, rehabilitation services, case management, social work, nutrition, and other consulting teams. Performing burn dressing changes at the bedside and assisting with dressing changes completed under sedation. Facilitating transitions of care and coordinating discharge planning and outpatient follow-up. Participating in multidisciplinary rounds and collaborative care planning. Medication Reminders Medication support is limited to the responsibilities authorized in the client’s approved non-medical care plan. Caregivers may: Provide scheduled reminders. Assist with an approved prefilled medication planner. Place approved medications in a cup when directed. Remind the client to self-administer their medication. Document reminders and client responses. Report refusals, missed doses or other concerns. Caregivers must not independently administer medication, change medication instructions or provide medical advice outside their authorized scope. Organization: VCH Endoscopy Job Summary: Responsible for planning, organizing, and administering patient care within the scope of his/her training and expertise, collaborating with other professional disciplines to ensure effective and efficient patient care delivery and achievement of desired patient outcomes is met. This is done in conjunction with the objectives and policies of the hospital and the Nurse Practice Act of the State of Tennessee. . Hours of Position: PRN, as needed. Analyzes comprehensive information pertinent to the patient’s care and management to determine nursing diagnosis.
