Duties include: patient check in/out, answering any questions that patient may have, and other related administrative tasks. a - Efficiently manages email communications in a timely manner, uses appropriately and remains current with division/organizational communications. b - Is proficient in the use of all computer applications, Meditech, lotus notes, i-heal.. c - Assists in uploading of photographs into Meditech and i-heal. 13 - Percept new employees using the established check off sheet reviewing departmental protocols, policies and workflow. a - serve as a role model/mentor providing support, training and sharing of knowledge including explaining and monitoring new workflows and policies 14 - Works in Collaboration with Nursing Supervisor in the preparation of yearly evaluations by providing individual feedback and assisting with chart review and reviewing yearly competencies and ensure compliance with all personnel policies. 15 - Commits to two sessions of our annual flu clinics 16 - Safety Awareness – Fosters a “Culture of Safety” through personal ownership and commitment to a safe environment a -Demonstrates required office and clinical equipment competencies necessary to successfully perform this role b -Verifies patient identification according to policy prior to administration of care and/or procedures. c -Complies with the current CDC hand hygiene guidelines through proper hand-washing. d - Adheres to universal precautions and makes appropriate use of personal protective equipment at all times. e - Appropriately disposes of hazardous materials f - Utilizes proper body mechanics when performing all aspects of the job g - Follows appropriate chain of communication/command. 17 - Other duties as required 18 - Works the required weekend and holiday schedule JOB REQUIREMENTS Minimum Education - Preferred High School Diploma or equivalent preferred Completion of a Medical Assistant program preferred. Drives beyond 120 miles as part of extended mileage may be required on special project days. Special projects may include an overnight hotel stay. • Obtains and maintains cross-state license in other states besides home state based on business need. • Collaborates with fellow nurse practitioners to develop best practices to perform work duties efficiently and effectively. • Actively participates in regional meetings. • May prescribe medications and perform procedures as appropriate. • Performs timely medical records documentation in electronic medical record (EMR) computer system. • On occasion, may be required to walk flights of stairs while carrying up to 50 lbs. of equipment. • Engages in practices constituting the practice of medicine in collaboration with and under the medical direction and supervision of a licensed physician to the degree required by state laws. • Local travel required (based upon state/contractual requirements). Presents the advantage of the hospice Medicare/Medicaid benefits to referral sources. 5. Thoroughly understands the Medicare/Medicaid hospice benefit, the Medicare home health benefit, the Medicare/Medicaid long term benefit and the Medicare HMO benefit. 6. Documents a narrative note in the EMR summarizing pertinent physical and psychosocial information from the hospital record and/or from the physician and patient/family assessment. Reviews referred patient’s history, medical status, and prognosis to determine eligibility for hospice services relying on the knowledge of hospital procedure and hospice admission criteria. 7. Completes the patient/family admission packet, including obtaining signed Notice of Election of Hospice Benefit (NOE), admission forms from the patient, family, referral source and the attending physician. Uploading completed admission documents electronically to medical records for them to complete their process. 8. Communicates information from Comprehensive Assessment to TMCAH CMO, hospice physician or TMCAH covering NP. Documents results and performs warm hand off to team. 9. Communicates NTUC to appropriate staff and managers. Documents reason and continuation of care measures implemented. 10. Promotes hospice services, to ensure appropriate referrals to hospice program, may perform liaison activities centered in the acute care setting, which include but not limited to, case conferences to case managers, discharge planners, physicians, other health care providers and referral sources. 11. Works with TMCAH liaison and intake staff to ensure ease of referral, as well as a rapid response to the referral source. 12. Evaluates and compares information, regarding referred patient to hospice criteria. Consult with either the referral management director, referral manager, CMO, or hospice physician when an admission is questionable. 13. Communicates on a regular basis with patient, family, referral source, hospice intake or liaison staff, to ensure continuity of care and adherence to the hospice plan of care. 14. Develops opportunities for and conducts in-service presentations in health care facilities regarding hospice care and the indications for a hospice referral. 15. Promotes knowledge and awareness of the hospice program and its services to the professional community. 16. Identifies potential new referral sources. 17. Joins approved professional organizations and other groups which include key decision makers in the care of terminally ill patients. 18. Assists with public relations activities to promote community awareness of Hospice services . Includes activities during National Hospice Month. 19. Reports Compliance concerns to the CEO or Chief Compliance Officer when applicable. 20. Pursues active membership in local and national professional organizations. Procedural Nurse Responsibilities Delivers expert, evidence-based nursing care to patients and families. Perform pre procedural assessment with patients by obtaining medical history via pre procedural phone call to provide patient specific instructions: Assess safety and tolerability of procedural sedation. Perform medication reconciliation. Review procedural sedation history. Provide pre procedural instructions, providing the patient with opportunity to ask clinical questions without requiring separate call back. Create patient specific premedication regiment in collaboration with procedure APP. Obtain IV access and preprocedural lab work. Administered preprocedural procedural medications, titrating according to specific patient needs. Also administer chemotherapy and performs procedures safely and efficiently in accordance with Institute policy. May administer blood transfusions. Work as co-sign to Intrathecal Chemotherapy administered by APP. Assists in emergency nursing procedures necessary for prompt control of changes in patient`s physical condition. Maintains awareness of procedural suite activities and works with the Procedure Suite team to maximize workflow. Keeps abreast of changing techniques in procedural nursing and maintains a high level of expertise in critical care. Responsible for intraprocedural monitoring of patients not limited to maintain active airway, assesses effective circulation and respiration via capnography and telemetry monitoring. Safely recover patients post sedation, perform discharge teaching, and evaluate safety of patient discharge. Coordinate correct research specimen collection with research coordinators. Maintain Joint commission readiness. Provide as needed support for biopsy specimen collection when not acting in a procedural sedation role. Knowledge, Skills And Abilities Demonstrated ability to work as an effective member of an interdisciplinary team. Demonstrated skills in critical thinking, problem solving, and ability to make independent decisions. Ability to function in a busy work setting with patients with complex needs. Minimum Job Qualifications Education: Associate degree, Nursing. Baccalaureate degree in Nursing strongly preferred. If Associate degree in Nursing, must complete BSN degree within 4 years. Experience: Completion of the Oncology Nurse Residency Program (ONRP) or 1 year of recent experience as a Registered Nurse. Ambulatory and/or oncology experience preferred. License/Certification/Registration Required Licensed as a Registered Nurse in the Commonwealth of Massachusetts, or State of New Hampshire as appropriate required.
