SUMMARY: The LPN is responsible for providing direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing to ensure that the highest degree of quality care is always maintained. ESSENTIAL DUTIES AND RESPONSIBILITIES: Direct the day-to-day functions of the nursing assistants. Meet with your assigned nursing staff, as well as support personnel, in planning the shift’s services, programs and activities. Make written & oral reports/recommendations concerning the activities of the shift as required. Admit, transfer and discharge residents as required. Provide leadership to nursing personnel assigned to your unit/shift. Complete & file required record-keeping forms/charts upon the resident’s admission, transfer and/or discharge. Receive & transcribe telephone orders from physicians & record on the Physician’s Order Form. Chart nurse’s notes in an informative & descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care. Fill out and complete accident/incident reports & submit to Director as required. Perform routine charting duties as required & in accordance with established charting & documentation policies & procedures. Prepare & administer medications as ordered by the physician. Develop & maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. Receive/give the nursing report upon reporting in and ending shift duty hours. Make rounds with physicians as necessary. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc., as required. Monitor your assigned personnel to ensure that they are following established safety regulations in the use of equipment and supplies. Maintain confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. Ensure that all nursing care is provided in privacy and that nursing service personnel knock before entering the resident’s room. Performs other duties as assigned. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must be able to read, write, speak, and understand the English language. Must be able to make independent decisions when circumstances warrant such action. Current CPR Certification is required Experience in Long Term Care is preferred. Ensures the patient understands said instructions. Initiates and follows up on cardiac and medical clearances for said tests/procedures. Prepares patient medical records for upcoming appointments to ensure all test results and/or outside records are obtained and ready for provider to review prior to the patient’s arrival for their scheduled appointment. Enters orders into electronic medical record (EMR)system as directed by physician. Faxes or electronically transmits/receives medical records for continuity of care. Completes Family and Medical Leave Act (FMLA) and Short and Long-Term Disability paperwork and coordinates with the Medical Information Department if additional medical records are requested along with said paperwork. Orders medical supplies and medications. Coordinates disposal of bio-hazardous materials with Johnson Memorial Health (JMH) and restocks patient exam rooms. Maintains cleanliness of patient exam rooms and nursing area. Maintains sample medication counts and logs. Works with the Pharmacy Department to properly dispose of expired medications. Performs instrument sterilization and proper maintenance of the autoclave. Coordinates incoming and outgoing referrals. Schedules appointments, completes registrations, scans insurance card(s), photo ID(s) and other pertinent information into the patient’s electronic record. Collects and posts co-payments and ensures that the appropriate insurance referral or authorization is received which allows us to consult and treat the patient for the condition for which they are scheduled. Runs insurance eligibility in Cerner and multiple websites to ensure the patient has active coverage and scans this information into Revenue Cycle system. Performs other related duties as assigned. Clearly communicates and continuously supports the Mission and Values of JMH. Conducts all activities in compliance with applicable laws, regulations, standards, and hospital policies and procedures including blood and body substance precautions. EDUCATION, EXPERIENCE AND TRAINING: High school graduate in addition to specialized courses to include the successful completion of, and graduation from, a recognized medical assistant training course or equivalent experience required. Current CMA (AAMA), RMA, NCMA or CCMA certification required. Requires current BLS or CPR for Healthcare Providers certification within 6 months of hire. At least one year of related Medical Assistant experience obtained in physician’s office or similar setting. JOB RESPONSIBILITIES: Assists in transportation of patients to and from the surgical services departments. Performs as a first assistant when assigned within scope of practice, competency, and skills. Reviews preference cards and makes proper selection of appropriate supplies and equipment for surgical procedures assigned. Accountable for supplies, equipment, and environmental support in a cost-effective manner. Drapes all equipment in a sterile fashion. Identifies specimens for processing by circulating nurse. Counts instruments, sharps, and sponges, before, during and after surgery. Cleans and prepares instruments for sterilization. Removes soiled instruments and equipment appropriately. Recognizes, intervenes, and reports situations which impede patient safety. Safeguards the privacy of patients and maintains confidentially. Maintains stock in Operating Room supply carts as needed. Recognizes and reports problems related to kits, equipment, or supplies. Contributes to cleanliness and order of the unit. Facilitates efficient turnover time by prioritizing activities related to equipment and instrumentation needed for cases to follow in assigned room. Ensures patient comfort and safety measures are maintained. Assists the surgeon during surgical procedures with a high degree of efficiency, anticipating physician and patient needs during the case. Assists in daily maintenance of the department, in completing work assignments, restocking, pulling cases for the next day and maintaining equipment in good working order. Utilizes non-chargeable items in a cost-effective manner. Orients and trains others on appropriate instruments/equipment use. Follows all appropriate procedures to decontaminate, sterilize, and disinfect equipment and instruments. Correctly assembles and wraps/packages items. Prepares items in an organized and timely manner. Operates the autoclave unit appropriately and performs high level disinfection processes. Performs other duties as required.
