Based on patient and agency needs, the nurse may also be required to perform follow-up visits. Home Health Admission RN Job Responsibilities: Provide skilled nursing care, including patient admission and discharge, in a home-based, family-centered environment while maintaining complete and accurate patient records in electronic and/or paper formats. Educate, demonstrate, supervise, and assess care provided by peers, Home Health Aides (HHAs), family members, and caregivers, fostering a therapeutic relationship that promotes self-advocacy and independence.
