Position Overview The Registered Nurse (RN) Supervisor plays a key leadership role in managing nursing operations during assigned shifts at Constant Care Agency’s partner nursing facilities in the Bronx. The RN Supervisor ensures that residents receive safe, high-quality care consistent with the standards of practice, regulatory requirements, and the mission of Constant Care Agency. This position requires strong clinical judgment, supervisory ability, and compassion for the elderly and long-term care population. Primary Responsibilities Clinical Supervision Oversee all nursing and direct care staff (RNs, LPNs, CNAs) on assigned units or shifts. Ensure timely and accurate delivery of nursing care in accordance with physicians’ orders, facility policies, and resident care plans. Monitor residents’ conditions; respond promptly to changes in status and initiate appropriate interventions or physician notifications. Conduct walking rounds at the beginning and end of each shift; verify continuity of care and accurate reporting. Oversee medication administration and treatments to ensure safety and compliance. Staff Leadership & Management Assign, direct, and evaluate the performance of nursing staff; delegate tasks appropriately based on scope of practice. Provide mentorship, education, and on-the-job training for new or less experienced staff. Ensure adequate staffing levels; manage call-ins and replace staff as needed in collaboration with the staffing coordinator. Promote teamwork, professionalism, and accountability across the care team. Regulatory Compliance Ensure nursing practices comply with New York State DOH , CMS , and facility policies . Conduct chart audits to confirm accurate, complete documentation of resident care and progress notes. Participate in QA/QI programs, infection control audits, and safety committees. Prepare for and participate in DOH inspections and surveys. Maintain resident confidentiality and HIPAA compliance at all times. Emergency Response Serve as the on-site clinical authority during assigned shifts. Respond to medical emergencies, perform immediate assessments, and direct staff response. Communicate critical incidents to the DON or Administrator on call. Resident & Family Relations Foster compassionate, resident-centered care and ensure residents’ dignity and rights are upheld. Communicate effectively with residents and families regarding care issues or changes in condition. Participate in care plan meetings when required. Qualifications Licensure: Current, unencumbered Registered Nurse (RN) license in New York State . Experience: Minimum 2–3 years of RN experience , with at least 1 year in a long-term care or skilled nursing facility . Prior supervisory experience strongly preferred. Education: Graduate of an accredited nursing program (BSN preferred). Certifications: BLS required; ACLS preferred. Knowledge: Comprehensive understanding of NYS DOH and CMS nursing home regulations. Familiarity with MDS documentation, infection control, fall prevention, and pressure injury protocols. Skills: Strong clinical assessment and problem-solving ability. Excellent leadership, delegation, and interpersonal communication skills. Ability to handle emergencies calmly and professionally. Proficiency in electronic health records (e.g., PointClickCare, SigmaCare). Work Conditions Work performed in a skilled nursing facility environment. Requires standing and walking for extended periods and occasional lifting up to 25 lbs. May require evening, night, weekend, or holiday shifts. Administer and monitor medications used to induce sedation in accordance with PPGNY protocols and RN scope of practice. Ensure safe, timely documentation, patient education, and post-sedation evaluation. · Accurately documents and administers medications as ordered by the provider and provides associated teaching. · Documents competently in the electronic medical record using the correct PPGNY templates and forms. · Arranges for interpreters for patients with communication barriers. · Collaborates with members of a multi-disciplinary health care team & provides care within New York State RN scope of practice. · Assists in promoting optimum center operations, excellent patient service and compliance with both agency and funding source policies and regulations. · Participates in health center/affiliate efforts to achieve established goals for productivity. · Adheres to affiliate goals and policies on professionalism, wait time in-clinic and on the phone, and the system for addressing client complaints. · Assists in the orientation and training of new RN/LPN or unlicensed medical personnel on the center team as needed. · Performs other educational and experience related duties as required. · Assists in abortion service in a variety of functions including abortion counseling, · Provides health education and nursing care related to family planning, reproductive health, abortion service, transgender health services, and STI’s to all clients. · Provide accurate patient centered, non-judgmental, education according to MS&G and signs informed consent related to visit. · Performs laboratory tests as ordered, completes laboratory requisitions, packages labs for transport according to PPGNY lab procedures. Performs daily controls per lab protocols. · Follows all safety and security procedures and guidelines. · Participates in agency infection prevention and quality control initiatives and protocols. · Notifies patients of laboratory results and educates regarding clinician plan for follow up. · Screens and follows up on patient phone calls in collaboration with center APC or PPGNY physicians. Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. This individual is also responsible for assisting with clinical, administrative and organizational tasks associated with patient care coordination. Responsibilities 1. Assists the provider during patient visit via obtaining vital signs (height, weight, blood pressure, pain scale etc) and documenting vital sign results and transcribing medication information into the electronic medical record (EMR). 2. Assists providers with procedures by preparing supplies and equipment and acting as a chaperone as needed. 10. Perform EKGs when ordered by provider 11. Performs pre-visit planning for Diabetic and CHF patients as per practice guidelines 12. Monitor and maintains refrigerator temperature logs and biomed expirations for medical equipment 13. Participates in daily huddles with MDs and other clinical team members to review daily care management patient lists and discuss patient's treatment plan & management 14. Performs age-specific screenings for Activities of Daily Living, Falls, Alcohol, Tobacco, and/or Depression screenings annually for every patient 15. Enters data into the electronic medical record related to medication adherence, diabetes control, foot/eye exam compliance for provider review and reconciliation 16. Assists providers with distributing educational materials to patients 17. Assists Population Health Management team by verifying patient has completed their colorectal screening, breast screening, cervical cancer screening and other pertinent preventive screens. 18. Retrieves information regarding hospital admission and Emergency Room visits from outside facilities for evaluation by provider for transition of care coordination 19. Assists providers with follow-up related to patients on Coumadin therapy 20. Assists at the front desk as needed: answer telephones, register patients, discharge patients, schedule appointments, copy charts, schedule ancillary tests, and follow up on overdue diagnostic test results. 21.
