***Position opens in the beginning of December, 2026*** Purpose of Your Job Position The primary purpose of your position is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility. Delegation of Authority As MDS Director you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. Job Function Every effort has been made to identify the essential functions of this position. However, it in no way states or implies that these are the only duties you will be required to perform. Duties and Responsibilities Administrative Functions Conduct and coordinate the development and completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs. Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the RAI process. Develop, implement, and maintain an ongoing quality assurance program for the resident assessment. Participate in Facility surveys (inspections) made by authorized government agencies. Monitor the Facility’s QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur. Ensure that a current copy of the RAI Manual is available to persons completing portions of the MDS. Committee Functions Chairperson for Daily Case Management Meeting. Ensure appropriate documentation to support services provided. Initiates, directs and maintains the Medicare PPS and OBRA assessment schedules to ensure timely completion of all assessments. Complete entry trackers and discharge assessments accurately and timely. Ensure that an Initial Admission Assessment is completed within fourteen (14) days of the resident’s admission. Ensure that quarterly and annual resident assessments are completed on a timely basis. Collaborate with the IDC team to identify significant change in resident status and, ensures that a comprehensive resident assessment is scheduled within fourteen (14) days of a significant change in the resident’s condition. Ensure that each portion of the assessment is signed and dated by the person completing that portion of the MDS. Sign and date the assessment instrument to certify its completion. Ensure that all assessments are completed and transmitted in a timely manner. Review validation reports for fatal errors and warnings and completes Modifications and Inactivations in accordance with CMS Correction Policy. Maintain a PPS pathway for all residents on Medicare Part A and Managed Care. Collaborate with facility Director of Rehabilitation to ensure the most appropriate ARD is utilized for Medicare assessments thru the use of PPS pathways Provide billers with PPS pathways to ensure accurate billing. Complete Comprehensive Status Updates to Case Managers on all managed care residents per company guidelines. Complete and maintain timely completion and signing of Physician Medicare Certifications. Prepare for State and Federal agency audits related to RAI process and reimbursement. Resident Rights Review complaints and grievances made by the resident and make a written and oral report to the Nurse Supervisor, LPN, and RN. Follow Facility’s established procedures. Maintain a written record of the resident’s complaints and/or grievances that indicates the action taken to resolve the complaint and the current status of the complaint. Must adhere to all HIPAA requirements. Working Conditions Works in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses’ stations, resident rooms, etc.). Is involved with residents, personnel, visitors, government agencies or personnel, etc., under all conditions and circumstances. Is involved in community or civic health matters or projects, as appropriate. Education Must possess, as a minimum, a Nursing Degree from an accredited college or university. Position Summary The Medical Assistant (MA) is a vital member of the Integra Vascular care team, supporting the clinical and operational success of our vascular programs. This individual serves as a key liaison between patients, providers, and facility staff—ensuring efficient communication, high-quality patient care, and adherence to compliance standards. The role involves both administrative and hands-on clinical responsibilities, providing support in our surgical centers, outpatient settings, and partner facilities. Key Responsibilities Clinical Support · Perform patient triage, obtain vital signs, and prepare patients for vascular procedures and consultations. · Assist physicians and nurse practitioners during procedures such as vein treatments, ablations, and diagnostic evaluations. · Prepare and maintain sterile fields, surgical instruments, and procedural supplies. · Ensure accurate documentation of all patient encounters, clinical findings, and procedural notes within the EMR system. · Provide pre- and post-procedural instructions to patients and caregivers, ensuring understanding of treatment plans and follow-up expectations. · Support the safe transfer, positioning, and monitoring of patients before, during, and after procedures. Operational & Administrative Support · Schedule and confirm patient appointments, coordinating across facilities and provider calendars. · Manage daily patient flow to maintain efficiency and minimize wait times. · Track and update care plans, referrals, and procedure outcomes for continuity of care. · Monitor and maintain clinical inventory, including restocking medical supplies and ensuring proper equipment functionality. · Assist with obtaining and uploading lab work, imaging reports, and diagnostic studies into EMR and PACS systems. · Communicate professionally with long-term care facilities, partner offices, and internal departments to ensure timely coordination of care. Compliance & Quality · Adhere strictly to HIPAA, OSHA, and Integra Vascular’s internal compliance policies and safety protocols. · Participate in quality assurance activities, reporting variances or potential risks to management. · Maintain up-to-date certifications and engage in ongoing clinical training and education. · Demonstrate commitment to patient safety, confidentiality, and a professional work environment at all times. Additional Responsibilities · Support other clinical staff and providers as needed during high-volume days or special procedural schedules. · Participate in staff meetings, training sessions, and process improvement initiatives. · Travel to various Integra Vascular facilities and partner sites as required. Required Qualifications · Active Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential. · Current Basic Life Support (BLS) certification. · Valid Driver’s License with a clean driving record. · Minimum 2 years of medical assistant experience, preferably in vascular, interventional radiology, or outpatient surgical settings. · Strong organizational, multitasking, and time-management abilities. · Excellent communication and interpersonal skills with patients and colleagues. · Proficiency with EMR systems; experience with PACS and medical imaging systems preferred. · Bilingual (Spanish/English) preferred but not required. · Must not be listed on the OIG Exclusion List. Job Requirements POSITION SUMMARY The Registered Nurse (RN) provides direct and in-direct patient care by adhering to the nursing process in a collaborative environment, promoting optimal patient outcomes through evidence-based practice, care coordination, and patient and family engagement. PRIMARY ACCOUNTABILITIES Provides safe and effective nursing care based on assessment, planning, implementation, and evaluation of patient needs across the care continuum. Administers medications, perform treatments, and document care activities in accordance with Health First’s policies and procedures. Collaborates with interdisciplinary team members to coordinate care, participate in rounds, and support discharge planning. Applies critical thinking to promptly identify changes in patient condition and initiate the appropriate interventions. Educates patients and families using teach-back to promote engagement in care and safe transitions. Adheres to safety protocols and infection prevention practices to protect patients and Associates. Utilizes Health First technology systems to enter clinical documentation and care plans. Participates in quality improvement, patient safety, and High Reliability Organization (HRO) initiatives. Supports precepting, mentoring, and onboarding of new Associates and students as appropriate. Work Experience MINIMUM QUALIFICATIONS Education: Associate’s degree in Nursing (ASN) or Nursing Diploma. Work Experience: Meets unit specific care requirements; new graduates taken under consideration based on the needs of the unit. Licensure: Registered Nurse (RN) licensure in the State of Florida or endorsement. Certification: American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card prior to start date and maintained. Additional certifications to meet unit specific care requirements (e.g., ACLS, PALS, NRP, NIHSS). Skills/Knowledge/Abilities: Strong clinical assessment and critical thinking skills. PHYSICAL REQUIREMENTS Physically agile; requires walking, standing, running, bending, stooping, kneeling, climbing, or crouching majority of the shift.
