As the Registered Nurse in Home Health you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. You will also coordinate total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of 'direct' and 'indirect' patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. Description The Offsite Licensed Practical Nurse I work as a part of the healthcare team by assisting in planning and implementing care interventions while promoting high quality patient care in a professional, efficient and safe manner, ensuring the patient flow. The Offsite LPN provides personal assistance, medical attention, emotional support, or other personal care to patients. This individual is expected to maintain up-to-date knowledge in this area and apply it to everyday practice. Responsibilities 1. Prepares and escorts the patient for exams and procedures. 2. Description The Nursing Attendant is an individual who functions in a support role to the Registered Professional Nurse by providing direct patient care and related activities as assigned. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations.
