Clinical duties will be in the Supervised Independent Living (SILP) for young adults currently in foster care in the Bronx, and two group residences: Kaplan House in Manhattan, and Cedar Knolls House. KEY ESSENTIAL FUNCTIONS: Work scheduled hours at two locations: Kaplan House and Cedar Knolls House. SILP youth will be seen in one of these group residences. Most work will be in-person, but some sessions may be provided via telehealth. Provide comprehensive primary care services, including detailed histories, physical exams and diagnoses. Incorporate emphasis on preventative care and medical screenings based on established guidance. Order and interpret diagnostic tests, using results to formulate treatment. Treat acute and chronic conditions, with appropriately scheduled follow-up and monitoring. Make referrals to subspecialist care when indicated, then incorporate subspecialist recommendations into comprehensive approach. Prescribe medication in accordance with best practices.
