Develop plan of care for resident with a current pressure sore, or evaluated as high risk for the development of pressure sores. Provides reports on the type and frequency of wound in the facility. Registered Nurse – Intensive Mobile Treatment Team (Full Time) About the Program Intensive Mobile Treatment (IMT) is a trauma-informed, community-based behavioral health program that provides treatment, rehabilitation, and support services to individuals who have not been successfully served by traditional systems. IMT teams deliver care directly in the community, including shelters, public spaces, and other natural settings. Position Overview The Registered Nurse (RN) serves as a primary practitioner responsible for assessing and monitoring participants’ clinical health and response to treatment. The RN works collaboratively with a multidisciplinary team including psychiatry, social work, peer specialists, and other staff to support participants in achieving their mental health and recovery goals. Key Responsibilities · Conduct psychiatric and physical health assessments for program participants. · Develop and coordinate care plans with the multidisciplinary treatment team. · Monitor participant health status and modify treatment plans as needed. · Administer medications, injections, vaccines, and perform phlebotomy as ordered. · Monitor vital signs and medication side effects and communicate findings to the treatment team. · Coordinate medical appointments and collaborate with psychiatrists and community physicians. · Provide treatment, rehabilitation, and recovery support services. · Advocate for participants and assist them in accessing community resources and services. · Maintain accurate clinical documentation, progress notes, and medical records. · Participate in team meetings, supervision, and required trainings. · Provide crisis intervention services on a rotating basis. Qualifications · Currently licensed as a Registered Professional Nurse (RN) by the New York State Education Department. · Minimum two (2) years of experience working as a Registered Nurse. · Minimum two (2) years of experience working with individuals with mental health conditions in a health or social services setting. · Strong written, verbal, and interpersonal communication skills. · Ability to assess situations, synthesize information, and make appropriate clinical decisions. · Eligible for full and unconditional participation in Medicaid and Medicare programs. · Computer proficiency including MS Word, Excel, PowerPoint, and electronic health systems. · Comfort providing services in community settings such as shelters, public spaces, and outreach environments. · Ability to travel within the community, including use of public transportation. · Bilingual Spanish preferred. · Must be fingerprinted and cleared by the New York State Justice Center. MAJOR FUNCTIONS: Under general direction, the Nurse Practitioner is responsible under direction of a physician for providing general medical care, preventative care and treatment to patients in one of the agency’s clinics. DUTIES AND RESPONSIBILITIES: Performs routine physical examinations and preventive health measures within prescribed guidelines and instructions of a physician based on assigned program. Orders, interprets and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs. Records accurate medical histories, physical findings and formulates plan and prognosis based on patient’s condition. Documentation includes medical evaluations, diagnosis, procedures, treatment, prognosis, referrals, education and consultation. Discusses case with physician and other health professionals and therapeutic team to prepare comprehensive patient care plan. Submits health care plan and goals of individual patients for periodic review and evaluation by physician. Prescribes medications and monitor effectiveness, or other forms of treatment such as physical therapy, inhalation therapy or related therapeutic procedures. Refers patients to physician or to specialized health resources for treatment. Makes patients aware of importance of a healthy lifestyle. Educates patients and their families on their preventive care, medical issues and use of prescribed medications and treatments. Analyzes new data gained from conferences, training workshops, professional literature and on the job training, where they will assimilate it into their clinical work. Keeps abreast of changes in field. Performs other duties as requested. REQUIREMENTS: Education, Experience and Skills Required: Master’s degree from an accredited advanced Nurse Practitioner program At least Two (2) years’ experience working as a Nurse Practitioner preferably in addiction medicine or substance use and/or mental health treatment programs Knowledgeable about federal and state regulations governing medical/clinical treatment in substance use treatment programs Ability to demonstrate sensitivity to patients with a variety of psycho social issues License/Certification Required: NYS RN and NP (Adult and Family specialty areas) licensures.
