Duties Description Independently performs the duties below: Conducts nursing assessments; and obtains, records, and updates individuals’ health information and vital signs, using standard instruments and procedures. Prepares, implements, and modifies nursing care plans; prepares individuals for and/or assists with examinations and treatments; and administers medications and nursing procedures to individuals, as prescribed Observes, monitors, and records individuals’ symptoms, conditions, and care, including diet and physical activity; and identifies and reports individuals’ responses to medication or changes in health status. Serves as the nursing representative on a treatment team; provides information on nursing needs; and consults and coordinates with treatment teams and other health care disciplines to assess, plan, implement, or evaluate individuals’ care. Makes clinical rounds to assess individuals’ nursing care needs and observe and participate in the provision of nursing services. Provides health teaching, counseling, and education to individuals, families, and other groups; and develops health improvement programs. Implements infection control protocols and procedures and provides related consultation and instruction to staff. Maintains and updates charts, records, and other documentation, consistent with agency, State, and federal requirements; and may collect data and perform related activities to support medical research. Monitors inventories and ensures that standards for medications, solutions, supplies, and equipment are maintained to meet nursing care needs. Provides clinical supervision and instruction to Graduate Nurses, Licensed Practical Nurses, and direct care staff in completing nursing care tasks. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. Acts as liaison to outside agencies and providers to ensure continuity of care and the necessary follow-up for individuals; advocates for individuals’ needs; and may provide instruction to providers on individuals’ discharge plans. May perform the full range of supervisory duties over Graduate Nurses, Licensed Practical Nurses, and direct care staff; provide information to Registered Nurse Supervisors; and prepare or contribute to the preparation of reports and performance evaluations for lower-level staff involved in nursing care activities. Description The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Participates in nursing orientation. 8. Interfaces with other departments in the Medical Center. Qualifications Education Requirements Bachelors Degree in Nursing; Masters preferred. Experience Requirements Previous experience as in homecare, long term care or utilization review preferred. Discharge Planner or Case Manager preferred. Manager or a minimum of 3-5 years experience as a RN in an acute care setting. Licensing and Certification Requirements: Current NYS RN nursing license. CCM certification preferred. Re-credentialing of CCM status expected. RN Supervisor needed- State-Of-The Art Nursing and Rehab Facility 3 - 11 & 11 - 7 Shift Full time Qualification: Current license as an RN in NY Minimum of 6 Month to 1 year experience as an RN supervisor in long term care Demonstrated competence as a nursing practitioner Demonstrated leadership and human relations skills Responsibilities: Responsible for oversight of the facility and ensuring compliance with federal and state regulations Maintains nursing guidelines are appropriate Establishes a compassionate environment by providing support to residents, families and employees. Maintains a safe and clean working environment Maintains resident confidentiality by monitoring confidential information processing. Maintains a cooperative relationship among the interdisciplinary team Coordinates the planning, implementing and evaluating of resident care. Makes daily environmental rounds - identifying and correcting problems. Submits an "RNS Checklist " to ADNS for each unit every month. Makes a daily clinical rounds seeing every resident on report and every resident on precautions. Schedules comprehensive rounds with charge nurse to see and evaluate nursing care on every resident on a regular basis. Oversees that grooming and personal care needs are met on all residents. Reviews, with charge nurses, all required documentation, nursing care plans and transcription of medical orders. Evaluates every new admission and assures nursing care needs are met. Evaluates every resident experiencing an accident/incident and reviews documentation of same. Evaluates every resident experiencing a change of status in health (including transfers to hospital) and reviews documentation of same. Monitors administration of medications and provides corrective in-service for nurses requiring the same. Monitors meals to assure compliance with policies. Assists in the orientation of new staff. Evaluates work performance of nursing staff. Provides disciplinary action and participates in grievances/arbitration. May immediately suspend and/or require staff to clock out and leave the facility for such unacceptable actions as drunkenness, fighting, patient abuse, refusing an assignment, insubordination, etc. Assists in the recruitment, selection, promotion, and termination of employment of nursing personnel. Assures adequate staffing for all units calling replacement staff as necessary. Completes investigation sheets on all accident reports and submits to ADNS. Monitors for adequate supply of oxygen and for proper storage, ordering additional oxygen as needed. Monitors emergency supplies and emergency drug boxes assuring readiness for emergencies. Communicates clearly shift-to-shift for continuity of care and reports to DNS through daily verbal reports.
