Safety: Supports a culture of safety and carries out safety interventions for all patients. 7. Infection Control: Adheres to all applicable infection control policies and procedures. 8. Sitter: Assist as patient safety attendant after completion of required competency. 9. Orientation: Assists with orientation of new Nursing Assistant colleagues. 10. Performs additional unit-based activities as assigned. B. Organizational Requirements: Incorporates Lahey Clinic Guiding Principles, Mission Statement and Goals into daily activities. The Home Infusion Nurse Coordinator teaches patients, care givers and home health care nurses to administer infusion therapy in the home and makes recommendations regarding safety to MD and inpatient Case manager. The HINC will coordinate with health care team members to assure timely progression of patients across the continuum of care and coordinates with home health care professionals to assure all involved are prepared to participate in home infusion therapy for the patient. The HINC maintains contact with patient, care givers and Health Care Professionals through assessments via phone weekly or as needed. Responsibilities Direct the daily activities of certified nursing assistants in accordance with current federal, state, and local regulations and guidelines, as well as established facility policies and procedures Ensure policies and procedures are complied with by nursing personnel assigned Participate in the admission, discharge, and transfers of residents as required Observe for changes in residents' status, notifying the physician and resident's family or representative, and documenting accordingly Transcribe physician orders to medical records and carry out orders as written Prepare and administer medications as per physicians' orders and observe for adverse effects Perform wound treatments as per physicians' orders, observe for changes, and document accordingly Schedules follow-up appointments for residents and transportation needs as indicated Provide nursing leadership to nursing personnel assigned to the unit Provide direct care skills such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc. in accordance with current policies and procedures Perform rounds to ensure resident needs are being met and personnel are performing their assigned duties Collaborate with other members of the interdisciplinary team as needed to ensure residents' needs are holistically met Inform appropriate departments of concerns or complaints and file grievances as necessary Report any incidents or unusual occurrences to the supervisor, unit manager, assistant director, or nursing director and participate in the investigative process as needed Participate in end-of-life care as required Ensure there is adequate stock of medications, supplies, equipment, and notify appropriate personnel of needs Initiate, review, and update care plans as required Promote teamwork, mutual respect, and effective communication Participate in the survey process when inspection surveys arise Requirements Current unencumbered State of Vermont RN license Strong communication skills to communicate with residents, families, and staff Critical thinking skills Ability to handle different situations with tact Benefit Package 6 Paid Holidays On-site free employee education Complimentary employee meals and more Medical Insurance Dental Insurance Vision Insurance Long & Short-term Disability Insurance Accrued paid time off – Full Time Employees 401K How to Apply If you are a motivated and compassionate healthcare professional looking for a new challenge, please apply to this role by submitting your application.
