Position Summary The Advanced Practice Provider (APP) in the CHF Clinic provides comprehensive, patient-centered care for individuals with congestive heart failure and related cardiovascular conditions. The APP works collaboratively with cardiologists, nurses, pharmacists, care coordinators, and other healthcare professionals to manage acute and chronic heart failure patients across outpatient settings. Responsibilities include patient assessment, diagnosis, treatment planning, medication management, patient education, and coordination of care to improve clinical outcomes and reduce hospital readmissions. Key Responsibilities Clinical Duties Conduct comprehensive patient evaluations, including medical history, physical examinations, and diagnostic interpretation. Diagnose and manage patients with heart failure and related cardiovascular diseases. Prescribe and manage medications, including guideline-directed medical therapy (GDMT) for heart failure.
