Job Summary The Nurse Navigator functions on the multidisciplinary team as an advocate, interpreter, educator, and counselor for oncology patients. Is responsible for ensuring all adult patients with an oncology diagnosis receive quality and comprehensive services, including coordinating patient care throughout the continuum in collaboration with the multidisciplinary team. The Nurse Navigator will serve as a clinical resource with expertise in oncology care management and will serve as a liaison throughout the facility and in the community regarding services provided for this unique patient population and will provide expert nursing care, which includes: direct clinical practice, consultation and education. Duties & Responsibilities Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance to the cancer patient from screening to survivorship. Systematically and continually performs the functions of assessing, planning, implementing, and evaluating the care according to the nursing process and Oncology Nursing Society Standards of Practice. Provides education and information to the patients and their family, helping to make the care seamless, continuous and comprehensive. Initiates and documents patient teaching including family and significant others based on assessment of needs, including psychosocial, and lifestyle practices. Responds to patient request for information regarding the disease process, expected side effects of treatment and community resources. Uses appropriate patient Education documentation modality. Supports the patient during difficult decision-making periods. Assists in coordination of end of life care for patient and family and provides emotional support. The Navigator partners with patients, families, interdisciplinary team, and community resources to provide well-coordinated, timely, compassionate, exemplary, interdisciplinary care and communicates with all members of the healthcare team, as appropriate about patient/family needs and concerns. Initiates and performs ongoing review of policies related to service provided and when appropriate, updates or writes new policies to enhance professional practice. Serves as a resource for community educational events, such as health fairs, screenings, symposiums, and lectures as well as support for staff education. Job Summary Under the direction of the Hyperbaric Supervisor provides direct care to Hyperbaric Oxygen Therapy patients as prescribed by the (Medical) Director Wound Recovery Center or attending Hyperbaric physician operates and maintains the HBOT chambers and associated systems and equipment and evaluates the care for clients according to age-specific criteria. The RN works within the State Practice Act and follows all Hyperbaric Medicine Wound Recovery Center and Kent Hospital policies and procedures. Duties & Responsibilities Operates maintains and ensures the safety of the hyperbaric facility and all associated systems and equipment prepares the chamber equipment and materials needed for patients undergoing HBOT. Carries out protocols of treatment as prescribed under physician’s direction observes patients and system during course of HBOT documents patient condition and reports unusual occurrences to HBO supervisor and Medical Director (physician on duty.) Maintains operating maintenance and safety logs and other required documentation performs preventive and other required maintenance (except yearly/biannual factory service) on the chamber/system as needed. Is responsible for cleanliness of equipment per policy assists/maintains a neat orderly work area appropriate inventory of supplies. Demonstrates competence in patient and family education including the purpose and procedure of HBOT. Maintains an active role in HBO education programs and orienting patient/family and visitors to unit. Participates in Quality Assurance and attends center meetings. Job Responsibilities Providing high-quality RN care in post-acute facilities Reviewing various care options with the medical team, residents, and their family members Coordinating with clinical teams to develop individualized care plans Being accountable to follow and adhere to resident care plans Administering appropriate medications and treatments to residents Drawing fluids as needed for labs and testing Using and monitoring medical equipment and devices to ensure effectiveness Training and supervising nursing professionals and other care staff consistent with scope of practice guidelines Promoting staff compliance with all applicable laws, regulations, facility policies, and nursing standards of care Documenting all relevant data to include vital signs, nursing interventions and responses, and resident histories Assisting staff members with incident investigations and the development of care best practices Qualifications and Skills Current Massachusetts state license to practice as a Registered Nurse (RN) Successful completion of an approved nursing program Minimum of 6 months of experience ( RN or related role) Advanced skills in multitasking, organization, and communication Ability to lift a minimum of 50 lbs Ready for a Job That Puts Your Needs First?
