RN Supervisor Long Term Care Facility

Position Summary: The RN Supervisor is responsible for overseeing nursing care and staff on assigned shifts to ensure the highest quality of care is delivered to residents. This leadership role involves clinical supervision, direct patient care, coordination with other departments, and maintaining regulatory compliance. The RN Supervisor supports and enforces policies, trains and mentors staff, and serves as a clinical resource within the facility. Key Responsibilities: Leadership & Supervision: Supervise LPNs, CNAs, and other nursing staff to ensure resident care meets standards and follows care plans. Act as the nurse-in-charge during assigned shifts, resolving clinical and staffing issues as needed. Monitor staff performance and provide guidance, coaching, and support. Assist with new staff orientation and ongoing training and development. Conduct shift hand-off reports and ensure continuity of care. Assist with scheduling and ensure appropriate staffing levels. Clinical Oversight: Perform and oversee comprehensive resident assessments and monitor for changes in condition. Administer and supervise administration of medications, treatments, and procedures. Communicate with physicians, families, and interdisciplinary team members regarding resident care. Ensure timely and accurate documentation in medical records. Respond to medical emergencies and initiate appropriate interventions. Support residents and families in understanding care plans and addressing concerns. Regulatory Compliance & Quality Assurance: Ensure nursing practices are in compliance with state and federal regulations, facility policies, and professional standards. Participate in audits, surveys, and quality improvement initiatives. Maintain infection control and safety protocols throughout the facility. Monitor and report incidents or changes in resident conditions per facility policy. Qualifications: Current RN license in the state of employment, in good standing Associate or Bachelor’s degree in Nursing (BSN preferred) Minimum 2 years of RN experience in long-term care or similar setting Supervisory or charge nurse experience required Current CPR/BLS certification (ACLS preferred, depending on facility) Strong leadership, problem-solving, and communication skills Proficient in Electronic Health Record (EHR) systems and clinical documentation Knowledge of nursing home regulations and resident rights Schedule: Days 7a-7p, Nights 7p-7a

Nurse Practitioner

Monday through Friday 8 to 5pm, no nights, weekends, or holidays! The Nurse Practitioner provides primary care to enrolled participants by evaluating participant physical complaints and providing appropriate treatment. The Nurse Practitioner functions beyond as an advanced practice nurse and as an integral member of the interdisciplinary team. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Evaluates participants and documents information in participant’s chart. Performs physical examination and orders and interprets diagnostic tests. Performs blood draws, EKG’s, dressing changes and any other nursing interventions as needed. Formulates an assessment of participant’s condition and provides a treatment plan, including writing prescription(s) and providing ongoing follow-up of condition. Provides alternative methods of therapy to participants as appropriate. Educates staff, participants and caregivers regarding healthcare issues and treatments. Manages acute and long-term medical needs of participants in collaboration with other primary care providers and oversees the care eds of participants in nursing homes, homes, Assisted Living Facilities and/or group clinic settings. Provides emotional and spiritual support to participants. Provides management of medical emergencies as needs dictate. Ensures appropriate and timely documentation of medical care in the participant’s medical record, in compliance with InnovAge Greater PACE documentation policies. Identifies documents and corrects problems in medical records.
