Overview Under direct supervision of a Registered Nurse or physician, the LPN performs nursing duties in the care and treatment of patients on an assigned unit. The duties require the use of tact and judgment in the care and treatment of patients. May assist in the supervision of non-professional subordinates. Is cognizant of philosophy, standards, objectives and policies of the Nursing Department. Performs related duties as required. Responsibilities Demonstrates proficient knowledge in the treatment of patients requiring nursing care. Uses available diagnostic results and other information to make an accurate evaluation. Applies knowledge of illnesses, injuries, and diseases and uses available resources in performing the nursing process. Performs timely, comprehensive, and accurate reevaluations and documentation at regular intervals as defined by the department, when a significant change occurs in the resident's condition, and when otherwise warranted. Is considerate of residents' privacy during the evaluation process. Contributes to an individualized and appropriate plan of care delineating measurable patient care goals Contributes to the plan of care making necessary changes as the condition of the patient indicates. Bases care decisions on resident needs and care priorities. Monitors patient outcomes. Monitor continued care needs for discharge and coordinates resources to meet post-institutional requirement. Collaborate, as appropriate, with Care Manager, physician, and other clinical disciplines in planning interventions to meet resident and family needs in a timely manner. Qualifications Education: Graduation from an accredited school of practical nursing or completion of equivalent study in an approved program in professional nursing. Experience: Minimum one (1) year clinical experience preferred. Description A Senior Nurse Practitioner is certified under section 6910 of the New York State Education Law and licensed as a registered nurse. A nurse practitioner/specialty coordinator provides clinical expertise and clinical program management for the provision of acute/primary health care to patients in accordance with written policies approved by nursing and medical administration. Responsibilities 1. Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement. 2. Description A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patient's condition that falls within their specialty area of practice. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. Protocols shall include provisions for case management including diagnosis, treatment, and appropriate recordkeeping by the Nurse Practitioner; and may include such other provisions as are determined by the Nurse Practitioner and collaborating physician to be appropriate. These protocols will be determined by area of specialty. 3. Practice Agreements Practice agreements shall include provisions for the following: a. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. However, consultation and documentation should be completed before more extensive tests are ordered, e.g., nuclear medicine tests, MRI, CT scan, IVP, etc. c. Consultation Nurse Practitioners consult with and refer patients to subspecialty clinics as needed. d. Prescriptive Practice Nurse Practitioners prescribe medications according to the New York State Laws and Regulations for prescription writing and record-keeping (see Appendix). e. Charting Nurse Practitioners are responsible for documenting findings, consultations and plans of care in the patients' medical records. .
