The Home Care Services Program (HCSP) is a Medicaid funded program that provides long term care to medically fragile and vulnerable individuals who require assistance with activities of daily living through the Community Alternative Systems Agency (CASA), Managed Long Term Care (MLTC) Program, the Assisted Living Program, Care at Home Program and Homebound Medicaid. The Medicaid Eligibility Unit is responsible for the Medicaid eligibility review and processing of initial applications, change actions, and renewals for all Medicaid funded long term care programs. This unit is responsible for the processing of Medicaid eligibility for consumers who receive services through one of the HCSP programs. Home Care Services Program (HCSP) is recruiting two (2) Staff Nurses, to function as Staff Nurses in HCSP/ CASA 11 – Care at Home/ Home Bound Unit, who will: - Conduct in-home or virtual assessments at the request of HRA using a State approved assessment instrument to evaluate the client’s ability to safely perform activities of daily living and the degree to which the client requires assistance for each function and task. The completed assessment would be entered into the New York State Department of Health (NYSDOH) and/or HRA computer system. - Assist in determining the level of home care services or other services of care that the client needs based on the medical diagnosis and degree of functional impairment. The development of a plan of care that takes into consideration client’s home environment and social support system. - Complete in-home and virtual visits and submit the completed HRA/NYSDOH approved assessments and recommendations forms no later than five (5) workdays after the date of assignment. - Conduct case reviews of new applicants, service change and re-authorization cases, and participate in special projects. The reviews consist of screening all documents related to the request for service and may include contacting the client’s physician or other resources for additional medical documentation and information as necessary. - Develop a plan of care for the type and amount of home care services to be provided to the client after reviewing all required case documentation. - Prepare documentation for and testify at Fair Hearings and at other legal proceedings to explain clinical findings and determinations regarding the plan of care. - Provide consultation to HRA Medical Review Team and social service staff at HRA’s request. - Complete self-audits of State approved assessments and medical review of charts. - Work toward obtaining PRI/screen certification and conduct assessments of clients transitioning to a Nursing Home Facility. Hours/Shift: 9AM – 5:30PM (1 Hr. Flex) STAFF NURSE - 50910 Minimum Qualifications 1. A valid New York State Registered Nurse License. Preferred Skills - Able to communicate well both verbally and in writing. - Able to efficiently utilize computer programs and enter data in an electronic field visit report. - Extensive UAS or home care assessment experience - Bilingual ability is preferred. - Strong clinical assessment skills. - Exceptional Time Management skills. - In-home assessments will require travel within the five boroughs of New York City regardless of what HCSP CASA site the Nurse is assigned to. Description A nursing student, who has completed the first clinical year, inclusive of medical – surgical nursing, within a baccalaureate nursing program and has a fundamental knowledge of the art and science of nursing, can function in this role. This experience assists the student nurse in expanding previously learned theory and skills through supervised patient care within a work setting. Responsibilities 1. Performs and records patient’s vital signs, such as blood pressure, pulse, temperature, respirations, pulse oximetry and pain rating 2. Documents clinical data as assigned, in a computer or manual system 3. Performs clinical procedures, such as specimen collection, venipuncture, EKG, application of sequential compression sleeves, post-mortem care, falls prevention, maintenance of skin integrity and capillary blood glucose monitoring 4. Sets up for selected procedures and assists RNs accordingly. 5. Assists patient with activities of daily living, such as feeding, bathing, and/or toileting. 6. Orients patients to Specialty Service/Unit environment. Assists patient with ambulation and all levels of mobilization 7. Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian 8. Description The Assistant Nurse Manager is a Registered Professional Nurse with broad knowledge and skills who acts as clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Clinical Nurse Manager. Responsibilities Patient Care 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. POSITION OVERVIEW We are looking for full-time Case Managers to work within a domestic violence residential & legal program. This program focuses of providing legal and job training programs for domestic violence victims, residential and vocational programs for individuals with developmental disabilities. The Case Manager is responsible for direct service provision to residents. Direct services include individual counseling, group counseling, advocacy services, referrals and follow-up, home management and budget counseling. REQUIREMENTS Conduct intake interviews, as assigned, to assess and evaluate residents for admission. Engage residents in initial and ongoing treatment processes, through individual and group counseling, under clinical supervision of the Superior of Clinical Service or Program Director. Develop and maintain housing referral resources, assist residents in the process of locating and securing permanent housing, in conjunction with Housing Specialist. Assess residents’ vocational and educational needs, aptitudes and interests through interviews and testing. Develop and maintain placement and follow-up as appropriate after assessment and counseling. Develop and maintain liaison activities with H.R.A. Public Assistance and Medicaid Administrators and Workers, to assist eligible residents in securing Public Assistance and Medicaid by acting as a personal advocate through telephone or personal visit/contact. Develop with the resident, a contract of mutually agreed goals and objectives to be accomplished within an agreed upon timetable. Facilitate psycho educational groups. QUALIFIFCATION AND EXPERIENCE Bachelor's degree in health/human services, sociology, psychology or social work. 2 years of relevant training and experience, including working with victims of domestic violence. Must be knowledgeable and sensitive towards domestic violence issues. Residential experience preferred. Overview Under direct supervision of a Registered Nurse or physician, the LPN performs nursing duties in the care and treatment of patients on an assigned unit.
