Keeps residents dry (changes clothing, linen, etc, when wet or soiled) Assists residents with bowel and bladder functions Empties urinary drainage bags and records amount in record; Reports any irregularities to nurse Assists with lifting, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., keeping with specific resident safety needs; Assists residents in ambulation, with or without self-help devices, as instructed Performs restorative and rehabilitative procedures, as instructed Ensures call lights are within reach of residents and answers call lights promptly Ensures residents’ personal care needs are being met in accordance with residents’ wishes Reports all changes in residents' condition to supervisor as soon as practical Changes bed linens and keeps linens tight to avoid wrinkles from forming under residents; Handles linens according to infection control guidelines Maintains cleanliness of resident units, central bathing area, treatment and supply rooms, workrooms, linen closets Assists residents in preparation for and transporting to appointments, activities and social programs Uses creative problem solving to meet the needs of the resident, within scope of responsibility Provides post-mortem care as instructed Performs required special nursing care duties, including identifying and reporting abnormal skin conditions Ensures residents are positioned and prepared appropriately for meals; serves food and assists with feeding as necessary; ensures each resident is properly hydrated; assists in the delivery and documentation of all snacks and supplements Accurately records all entries on flow sheets, I and O's, and other documents at point of care as required Participates in care conference as indicated and follows individual resident care plans Performs all admission, discharge and transfer duties as required Maintains confidentiality of necessary information Utilizes supplies and equipment properly and without waste, including restocking supplies Reports observations concerning structural, equipment and furniture defects and malfunctioning to appropriate personnel Handles unusual occurrences calmly and logically to maintain continuity of duties Maintains appropriate, effective communication with residents, and with residents' family or other significant relationships, and ensures departmental staff does the same Follows all appropriate safety and security guidelines, care procedures and protocol for residents in the Dementia Care Unit Participates in relevant educational and training activities as appropriate Participates in special activities as necessary (e.g., safety committee; mentoring/orientation/new hires; etc.) Label and charge ancillary items for resident billing Performs all other duties as assigned or directed Competency Statement(s) Accountability - Ability to accept responsibility and account for his/her actions. Assists the Executive Director/Administrator in the preparation and administration of the organization's budget. 7. Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues or expenses. 8. Evaluates performance of Clinical Managers. 9. Assists Clinical Managers to develop skills and techniques in evaluating the performance of clinicians. 10. Hires, evaluates, and terminates organization personnel. 11. Conducts annual evaluations on clinicians, or more frequently if indicated. 12. Oversees the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities. 13. Assures proper maintenance of clinical records in compliance with local, state and federal laws. 14. Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services. 15. Develops, implements and evaluates the orientation program for new organization personnel. Responsible for orientation of new organization personnel, directly or delegated. 16. Plans and implements inservice and continuing education programs to meet education and training needs of organization personnel at least quarterly. 17. Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of hospice services provided through the Organization. 18. Assists in the development of organization goals. Develops, recommends, and administers Organization policies and procedures. 19. Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance to the CHAP Accreditation standards. 22. Stays informed about changes in the field of nursing and hospice care; shares information with appropriate organization personnel. 23. Promotes hospice referrals in the health care community. 24. In the absence of the Executive Director/Administrator, the Clinical Director becomes the acting Executive Director/Administrator and will be vested with authority to act in behalf of the Executive Director/Administrator. 25. Other duties as assigned by the Executive Director/Administrator. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice in the state of Ohio. 3. Either a Bachelor’s degree in nursing with three (3) years of management experience within the last five (5) years in a hospice or related health care organization, at least one (1) year of which was in a supervisory or administrative capacity, OR A Registered Nurse with four (4) years’ experience within the last five (5) years in a hospice, home health agency, primary care clinic or health facility, at least one (1) year of which was in a supervisory or administrative capacity. 4. Once an offer of employment is made, it is contingent upon satisfactory references, as requested, and criminal background checks by regulation. 5. Demonstrated ability to supervise and direct professional and administrative personnel. 6. Ability to market and deal tactfully with customers and the community. 7. Position Overview: The Night shift Supervisor is responsible for directing the operation of a nursing unit in a long-term community. Key Responsibilities: Perform general management functions such as hiring, disciplining, and evaluating care team members. Plan and facilitate meetings and committees to address resident care issues/concerns for the unit. Manage area of responsibility with the goal of achieving and maintaining the highest quality of care possible. Participate in developing, implementing, and evaluating programs that promote the recruitment, retention, development, and continuing education of nursing staff members. Contact physicians as needed/necessary and ensure physician interventions are timely and appropriate. Review staffing patterns and census of nursing units and reassign personnel when necessary to ensure staffing meets resident needs and budgetary controls. Monitor the clinical operations of the unit and resident’s conditions and ensure that appropriate and quality care is administered. Obtain medications, supplies and medical records needed to provide safe, efficient, and therapeutic are to residents on a continuing basis. Assist in the orientation of new personnel, monitoring their skills and guides and observe care team members that may need assistance with procedures. Qualifications: Graduate of accredited school of nursing. Licensed as an RN in the state employed; license must be active, valid, and in good standing. Valid CPR teaching certificate preferred, but not required. Must hold an active CPR license or the ability to obtain within the first 30-days of employment. CPR license must be kept active and current throughout employment. Three years of nursing experience, of which one year in a long-term care environment.
