Nurse Practitioner - Complex Care Services Department Summary Advanced Practice Registered Nurse (APRN) to function in the inpatient Complex Care Service (CCS) at Boston Children’s Hospital. CCS is a long-standing and nationally regarded inpatient service in Pediatric Hospital Medicine within the Division of General Pediatrics that serves hospitalized children with medical complexity, who have complex chronic conditions and functional limitations. Common reasons for admission to CCS include infections (aspiration pneumonia, pyelonephritis), chronic condition exacerbations (epilepsy, digestive dysmotility), and post-operative recovery following surgery (baclofen pump placement, gastrostomy, spinal fusion, etc.). This role will provide patient evaluation, assessment, and facilitate care for the patients on the CCS service and will implement interventions tailored to improve systems for patient care and enhance patient safety for this population. The CCS APRN will work collaboratively with supervising physicians, pediatric trainees (residents and fellows), and the entire nursing team to promote optimal clinical care. This APRN will act as a clinical resource, facilitating staff educational opportunities. The setting is fast-paced and collaborative with many opportunities for learning. Job Summary: Hours: Part-time, 24 hours per week, weekdays and weekends, shifts will vary: 6am-2:30pm, 8:00am-4:30pm, 12:00pm-8:30pm & 7:00am-8:30pm Union: Yes Union Name: 1199SEIU Patient Facing: Yes Boston Health Care for the Homeless Program is looking for a Patient Benefits Coordinator/Medical Assistant to provide clinical and clerical support services at the Southampton Street Shelter and Transitions Shelter. The position is crucial to assure the continuity of patient care, providing an essential link for communication involving all departments and including transportation. Responsibilities: Clinical Support : Under the direction of the Site Manager, RN Clinic Manager, and/or Charge Nurse, performs a variety of appropriately delegated patient care interventions Collaborates with the supervising RN throughout the shift to determine appropriate work assignments and priorities, as well as communicating the status of delegated task completion and outcome of patient data findings. Maintains timely ongoing verbal communication in order to assure good patient care and the effective running of the Clinic Responsible for collecting, collating, and entering data and information into the applicable database or format in an accurate and timely manner; maintains appropriate equipment logs and records per standards Accurately collects requested patient date (e.g. vital signs, weights and reason for visits), notifying the supervising RN of abnormalities; performs basic patient testing, including, but not limited to, tests for guiac, urine dip, and glucose fingerstick Treats patient in a manner that preserves and supports patient confidentiality, patient rights, and patient’s dignity; act as a member of the team of care givers Collects, labels, and sends specimens to the lab, with appropriate laboratory slips and/or requisitions Assists nurse/provider during complex treatments/procedures, including but not limited to, doing preps and set-ups for examinations/treatments, assisting during procedures when requested or required Check inventories on a weekly basis, order clinic supplies and stock them in their appropriate location upon arrival in order to avoid a shortage or misplacement of necessary supplies Clerical Support: Work effectively with BHCHP administrative and clinical staff to obtain information or paperwork necessary for enrolling patients into the appropriate medical or cash benefits programs. Copying, faxing and filing as needed Greet and register all patients Phone coverage and triage of calls Scheduling and recording primary and specialty appointments correctly and in a timely manner Handle third party billing process Qualifications: Successful completion of a Certified Medical Assistant program High School Diploma or Equivalent required Phlebotomy required Experience in a medical setting preferred Secretarial training a plus Knowledge of medical terminology and vital signs Computer skills preferred, including MS Office, MSI, IDX, Logician, EPIC Ability to operate basic office equipment (fax, copier) Compensation and Benefits: The compensation increases based on years of experience and ranges from $22.25 -$35.60 per hour. Job Description: I. Position Summary: The primary role of the Patient Care Associate (PCA), is to protect, sustain, and nurture the patients that live at the Center. They provide assistance with activities of daily living, care of the patient’s environment, and meet other needs as required. The goal of the work of the Patient Care Associate, is to provide assistance that promotes a high quality of life for the patients. The Patient Care Associate, works as a member of a team that seeks to achieve a home like setting while maintaining excellence in all areas of care. Position Responsibilities: 1. Recognize and respond to the needs of the patients and families 2. Recognize and communicate changes in the patient’s condition to the appropriate nursing team member 3. Assist patients with all aspects of personal care as indicated in the care plans 6. Document pertinent information regarding care delivered in the designated areas of the patient’s electronic medical record 7. Report accidents and incidents as they occur 8. Report any observed or suspected patient abuse immediately 9. Observe and report skin integrity 10. Participate in daily report team and care planning meetings as needed 11. Be proficient in obtaining vital signs and other procedures such as temperature, pulse, respirations, blood pressure, weight, pulse oximetry and urine and stool specimens as directed 12. Follow infection control practices 13. Attend and participate in ongoing training and educational classes 14. Maintain confidentiality and privacy of the patient according to HIPPA regulations 15. Communicate effectively with peers, other team members, patients and families 16. Facilitate life enrichment activities that encourage patient engagement 17. Qualifications : Long term chronic care PCA Certified Nursing Assistant certification is preferred in good standing upon hire or current enrollment in a nursing school, having completed first clinical experience.
