Job Description The Department of Ambulatory Care Services at SUNY Downstate Health Sciences University is seeking a full-time Licensed Practical Nurse, Suite H. Distinguishing Characteristics Licensed Practical Nurses are licensed health care practitioners who provide direct nursing care to individuals in a variety of settings under the direction of a registered nurse, physician, or other licensed health care provider, in accordance with Article 139 of the Education Law. Incumbents perform nursing care tasks and procedures consistent with individual treatment plans; administer medications as prescribed; observe and report individuals’ responses to treatment and changes in health status; attend to individuals’ personal needs, such as activities of daily living and hygiene tasks; and provide emotional support and health teaching to individuals. Illustrative Duties Under the direction of a registered nurse, physician, or other licensed health care provider, performs nursing care tasks and procedures consistent with individual treatment plans and scope of practice, to maintain health, prevent and cure illness and injury, and alleviate suffering using practical knowledge of health processes and facility procedures. Assists registered nurses, physicians, or other licensed health care providers in the identification of individuals’ needs and participates in the development of plans to address goals. Administers medications and nursing procedures to individuals as prescribed. Obtains and records individuals’ temperatures, blood pressures, pulse rates, and other vital signs using standard instruments and procedures. Observes, monitors, and records individuals’ symptoms and conditions to obtain and provide information for diagnosis and treatment; identifies responses to medication or changes in health status; and reports such to a registered nurse, physician, or other licensed health care provider. Prepares individuals for or assists with examinations and treatments. Maintains charts and other records of individuals’ conditions and/or administered treatments. Attends to individuals’ personal needs, including activities of daily living and hygiene tasks, such as bathing, dressing, feeding, and grooming. Talks to individuals, listens to their concerns and responses, and encourages them to cooperate and participate in treatment and rehabilitation activities. Educates and counsels individuals and/or families regarding their health, medications, treatment goals and procedures, and behavioral interactions as appropriate. May perform housekeeping tasks, such as changing linens, and assist other staff in cleaning activities to ensure a safe, clean, and therapeutic environment for individuals. Monitors supply inventories and completes requisition forms to ensure adequate supply levels. May observe and supervise the activities of unlicensed direct care staff, including the administration of medication, to ensure compliance with applicable requirements; and reports observations to higher-level staff. Qualifications Required Qualifications Open Competitive: eligible for or possession of a license and current registration as a Licensed Practical Nurse in New York State. 55 B/C Qualifications Individuals certified eligible for the 55 B/C program under the Governor's Program to Hire Persons/Veterans with Disabilities pursuant to Section 55 B/C of the Civil Service Law will also be considered. V Home Infusion Nurse (($55 AN HOUR STARTING RATE!) Job Description UniversaCare is seeking to hire PRN Home Infusion Registered Nurse. Position: Per-diem Hours: Flexible Hours, Evenings & Weekends Start Date: Immediately As a Home Care PRN Infusion Registered Nurse (RN) , you will experience a special energy and excitement from your work as you join a team of individuals who are committed to improving the lives of others. You will work in a growing, specialized area of the nursing field outside the walls of a hospital setting As a Home Infusion Registered Nurse you will provide a broad range of infusion therapies in out patients’ homes while: Compassionately taking care of patients in order to allow them to recover in the comfort of their own home by using your clinical IV capabilities and personal touch. Skillfully providing one-on-one home infusion nursing care visits and patient education in accordance with the physician’s plan of treatment. Assessing the patient’s condition and response to therapy, taking action as needed to facilitate achievement of treatment goals. Establishing trust and building therapeutically effective relationships with our clients and their families. Required Qualifications: Active and unrestricted registered nurse with current license in state of NY Two (2)+ years of experience working as an RN in a Critical Care setting that contributes to proficiency in infusion therapy (such as medical surgical unit, ED, ICU, post-anesthesia care, or similar setting) Experience with lab draws, IV infusions, and reconstitution of medications Strong experience with working with a variety of pumps (CAAD, curling, etc) & troubleshooting issues that may arise Possess a valid and current driver’s license and auto insurance/registration Ability to handle specialty cases like IGG, IVIG, Solumedrol, etc. Have and maintain current CPR / BLS certification Knowledge of HIPAA patient privacy and data security requirements; OSHA employee protection regulations Hold current Malpractice Insurance Have experience with Microsoft Office to document and track patient care activity About Us: Founded in 2008, UniversaCare is a fully licensed and growing Home Care Agency serving the NY Metropolitan area and is continually expanding throughout the Northeast.
