Description RN/Case Management MSH Case Management FT Weekdays/Weekends/Holidays The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Participates in nursing orientation. 8. Interfaces with other departments in the Medical Center. Qualifications Education Requirements Bachelors Degree in Nursing; Masters preferred. Experience Requirements Previous experience as in homecare, long term care or utilization review preferred. Discharge Planner or Case Manager preferred. Manager or a minimum of 3-5 years experience as a RN in an acute care setting. Licensing and Certification Requirements: Current NYS RN nursing license. CCM certification preferred. Re-credentialing of CCM status expected. Description Job Title - Remote Nurse Practitioner, Palliative Care at Home - Mount Sinai Home Health - Full Time - Day Palliative Care at Home, part of Mount Sinai at Home, provides home based palliative symptom assessment, treatment and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team is comprised of CHWs, NPs, nurses, social workers and physicians. The team works in collaboration with patients’ healthcare teams to provide high quality evidence based complex palliative care and chronic care management and is designed to improve symptoms and QOL, reduce caregiver burden, reduce unnecessary inpatient utilization, and facilitate hospice when appropriate. Nurse practitioners will be responsible for the symptom management of chronically/terminally ill patients within an interdisciplinary team environment. This role will provide disease process and trajectory education to the team, patients and families. The NP will assess, order and interpret clinical testing, diagnose and treat symptoms related to a variety of chronic disease states to focus on palliative symptoms and advanced illness education. Responsibilities DESCRIPTION OF COMPETENCIES A. Operational Duties • Conducts in home and telehealth assessments for patients with complex needs, including formulating a diagnosis, ordering and interpreting diagnostics, and delivering appropriate care in line with patient preferences and goals of care • Prescribe new medications and perform medication reconciliation • Demonstrate sound clinical judgement and evidenced based practice in symptom management and end of life care • Facilitate family meetings, goals of care conversations, and ACP discussions • Leads oversight of care plans in conjunction with the rest of the clinical care team • Advocates for the patient and caregiver in collaboration with other care providers to keep care coordinated and consistent with patient goals • Serve as a link between health and social care services and community to improve access to palliative care and quality of care • Work closely with interdisciplinary team to address changes in patient needs or symptoms • Educates patient/family on trajectory of illness, use of medications, assesses disease understanding • Participate in weekly IDT meeting with care team • Excellent teamwork, communication and leadership skills • Documentation as required • Performs other related duties as assigned Qualifications Required: Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patients condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-master's NP certificate. • Licensure: New York State License with current registration as a registered nurse. • State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: • a. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. • b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). • c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR • Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Please note all NPs regardless of their status will be placed on an FPPE initially) Experience: Previous progressive experience relevant to the area of clinical practice required. For all NPs hired effective June 1, 2019, National Board Certification in Advanced Practice Nursing required. For NPs who bill for professional services, National Board Certification is required regardless of hire date. The Board Certification requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee. Description The Medical Office Specialist offers clinical and medical office support to physicians and surgeons. Assists in patient care and administrative duties necessary to deliver quality patient care. Performs various duties as a member of the health team in the treatment and care of patient and their families and also assists with tasks to support the unit. Participates and supports the health team in the direct patient care under the supervision of an RN/LPN or MD. Responsibilities Monitors patient flow in waiting room and resolves patient flow issues related to registration and post-visit processing Provides set-up of examination room and instruments in accordance with proper techniques established by hospital and infection control policies. Busy Adult Day Care Center is seeking a Certified Nursing Assistant. Job Responsibilities: 1. Provide all care as assigned within scope of practice for CNAs. 2. Maintains infection control practices, universal precautions, contact precautions, airborne/droplet precautions, and handwashing per facility protocol. 3. Practices the highest level of customer service in dealing with all internal and external customers, and fosters the same all departmental staff and peers as applicable. 4. Assist resident with any and all activities including but not limited to Meals, rehab program and activities. appropriately and clean linen/clothing is put away. 5. Assists in emergency procedures ie, code E, fire, disaster, and general health care emergencies. as directed. 6. Give post-mortem care and assists in transportation to morgue. 7. Assists in therapeutic activities as directed by Nursing Department. 8. Observes and reports all findings and/or changes of condition of patients/residents promptly to nurse in charge. 9. Completes required documentation daily. 10. Assist in orientation of new or recently transferred staff 11. Follows code of conduct and ensures resident privacy and is compliant with HIPAA. 12.
