A Brief Overview The Pediatric Progressive Care Unit is a 24-bed unit within the critical care continuum that manages medically complex, mechanically ventilated, tracheostomy-dependent patients that are transferred from the intensive care units. This unit provides a nationally recognized education and training program that supports technology dependent children's discharge to home. The Clinical Nurse practices at the level of an advanced beginner to expert practitioner of professional nursing in accordance with, and guided by the CHOP Nursing Professional Practice Model. The nurse at this level demonstrates competence in caring for patients and families and progresses to caring for those with the most complex problems/highest acuity in the designated clinical area. Provides age/developmentally appropriate care. Care is delivered through RN leadership for patient care within the framework of family-centered care. The Clinical Nurse adheres to the organizational and nursing department mission, vision, and values. Clinical Nurses practice in accordance with the ANA Code of Ethics; the ANA Standards of Clinical Nursing Practice; practice standards of applicable specialty nursing organizations; and CHOP developed standards of nursing practice. Job Description Nemours is seeking Nursing Assistant to join our team in Wilmington, DE for our Short Stay Observation Unit. Position is full-time, 60 hrs every two weeks, rotating shifts, with weekend and holiday rotations. 4 West is a 24-four-bed limited-stay unit that typically cares for general medical surgical patients whose care is driven by protocols. The unit consists of single patient care rooms, which can also be isolation rooms. The goal of the unit is to provide very high-quality, high-value patient care within a 72-hour time frame. Each patient room is equipped with oxygen, compressed air and suction, along with state-of-the-art cardiorespiratory bedside monitors and computers that enable the health care team to access a patient’s electronic medical record. These computers are also used in the barcoding administration of medications which ensures patient safety by verifying that the correct medication is given to the appropriate child. Family-centered care rounds are performed daily to assure an interprofessional approach and partnership with the patient and family The nursing assistant role is responsible for assisting with general activities of the patient care unit and assist the health care team with performance of patient care activities, under the direction and guidance of a Registered Nurse. Essential Functions Ensures an adequate stock of all necessary equipment and supplies in designated areas at established par levels. Ensures a clean and safe patient care unit. Assists patients with activities of daily life (bath, feed, play, etc.) and provides other patient care as directed. Assures all supplies are accurately charged. Contacts pharmacy, dietary, and other departments as designated by the Registered Nurse. Collects patient specimens (blood, urine, stool, other per policy), paying close attention to patient safety, comfort and accuracy of information. Job Description Nemours Children’s Health is seeking an experienced pediatric telephone triage nurse to support our primary care practices in Delaware, Pennsylvania and Florida. This is a fully remote position that requires every other weekend and every other holiday. This is a full-time role with 30 hours a week schedule and potential for up to 40hrs/wk. Weekdays Hours for the role range from: 7:00am-5pm (D) 4:00pm-12am (AH) Overnight 12:00pm-8:30am No permanent night shifts are included for this role, but must be available to cover 1 night per every 8 weeks Weekends 7:00am-3:30pm 3:30pm-12:00am Primary Function The Triage Nurse role provides provide primary care patients telephone access to a highly skilled and trained registered nurse who accurately answers their questions and addresses their medical concerns during hours when the primary care offices are open. To provide first line response to patient questions and concerns with escalation to the primary care offices when necessary. To ensure continuity of care, communicates the patient’s condition and plan of care to other health team members. Essential Functions Accurately assesses each patient’s condition by actively listening to callers and asking probative questions. Identifies emergent situations and effectively transfers life threatening calls to appropriate agencies per protocol. Provides appropriate and courteous telephone advice including plan of care and patient education to callers according to approved protocols. Appropriately and accurately documents initial and ongoing telephone conversations with documentation being completed immediately after the call ends. Places outbound follow up calls to caregivers as needed. Accurately communicates information to physician on call as needed either over the telephone or by routing the necessary information to the physician through EPIC. Works autonomously while adhering to organizational standards and core values. Attends and participates in staff meetings and required departmental meetings. Meets the needs of the department by flexing hours as needed based on seasonal changes and call volume. This may include morning, evening, weekend, and midnight shifts. Meets unit expectations for call volumes, call duration and customer service expectations. Appropriately utilizes and maintains Nemours supplied equipment; ability to troubleshoot technology issues to ensure uninterrupted service to patients. Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee’s supervisor. Job Requirements Bachelor’s Degree strongly preferred. Associates Degree prepared candidates must be actively enrolled in a BSN program. Upon hire, the Kids Health On Call nurse must have an active unrestricted Delaware Registered Nurse license to start. Active Pennsylvania and Florida Registered Nurse Licenses must be obtained within 90 days of hire date. BLS from American Heart Association required for all nursing roles. Nurses residing in an interstate compact state (DE, NJ or MD) who have a multistate license for their state of residence will not need to obtain separate PA or FL licensure. Nurses residing in PA will continue to require separate DE and FL licensure until PA begins full participation in the interstate compact. The KHOC associate must not work autonomously until all licensure is obtained and verified- they can shadow and train with a KHOC preceptor, but cannot provide medical advice for patients receiving primary care services in states for which the associate has not yet obtained licensure. 3-5 years of job related experience.
