Responsibilities The LPN/LVN is responsible for clinical functions on the unit, including all aspects of patient care incorporating the nursing process in accordance with the applicable Nurse Practice Act. The staff LPN/LVN shall ensure that the unit to which assigned shall function in a smooth, organized, and efficient manner as directed by the Registered Nurse. The provision of care includes direct care along with the delegation and supervision of all nursing care. Specific components of the licensed practical/vocational nurse role include demonstrating clinical competence, managing patient care, improving quality of care, establishing professional relationships and utilizing professional skills, fulfilling responsibilities of the role of the hospital, and developing clinical expertise per specialty. Required Skills: Current, valid, and active license to practice as a Licensed Practical/Vocational Nurse in the state of employment required. Current BLS certification from a Vibra-approved vendor required. IV Certification required. Additional Qualifications/Skills : One (1) year of clinical experience in a health care setting preferred. Three (3) years medical/surgical experience preferred. One (1) year of area specific experience preferred. Ability to project a professional image. Knowledge of regulatory standards and compliance requirements. Strong organizational, prioritizing and analytical skills. Ability to make independent decisions when circumstances warrant. About this unit 8NE & NW, is a 68-bed unit cardiac step-down unit that provides an intermediate level of care to patients that typically have a cardiac related diagnosis such as coronary artery disease (CAD), congestive heart failure (CHF), myocardial infarction (MI) and heart arrhythmias such as atrial fibrillation (A-fib) and ventricular tachycardia (V-Tach). We also provide pre and post procedure care to patients that have undergone cardiac procedures, such as catheterization/angioplasty, stents, ablations, and placement of pacemakers, and defibrillators. Our nurses receive specialized training in arterial sheath removal and Tikosyn administration and are ACLS certified. All patients are continuously monitored by telemetry. About the u nit We are a 64 bed Inpatient Rehab Unit that sees a variety of patients who need to work on both their functional and medical problems. We treat ortho patients recovering from hip fractures, complex knee replacement, back surgery with neurologic involvement, multi-trauma, and other orthopedic issues. We also see neurology patients recovering from stroke, brain and spinal cord tumors, Traumatic Brain Injuries, Parkinson’s, MS, and any cancer diagnosis with functional decline. We also have patients come to our unit who had amputations and other complex medical/surgical issues with functional decline. Patients participate in 15 hours of therapy across the 7-day week. Nursing continues the interventions practiced by the patients in therapy sessions, to continue safe mobility. For many of our patients, Rehab is the key to getting back home with their families. This is a very rewarding unit to work on, as we all get to take part in the progress patients make towards their goal of getting home. This role is a team based role as a hospitalist APP, working with the hospitalist physicians at our Trenton, Taylor and Wayne campuses. The duties include, but are not limited to admissions, daily rounding, discharges, addressing nursing and family concerns, family education, precepting students, and maintaining the high quality care given to our patients. Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up. Writes prescriptions for medication, blood products based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols. Determines eligibility of patients for entry into clinical trials based on findings. Requests written consultation from physicians and other healthcare professionals to ensure appropriate and quality patient care. Interprets and evaluates findings of studies/tests. Relays appropriate information regarding patient care to the collaborating physician. Administers therapeutic measures and obtains specimens as prescribed. Evaluates the quality of care provided and recommends changes for improvement. Initiates appropriate actions to facilitate the implementation of therapeutic plans that are consistent with the continuing healthcare needs of the client. Dictates follow-up letters to referring physicians with summaries of treatment, response, and plan. Maintains provision of preventive health services (e.g. screening, risks assessment, immunizations, PAP smears, self-breast exam). Dictates or writes patient history, admissions, care plans, progress, and discharge notes. Interacts with program assistants and data managers for optimal patient care. Assists in the management of family dynamics and coping mechanisms during acute and chronic phases of patient care. Conducts or participates in daily multidisciplinary rounds and ensures appropriate and quality care for assigned patient. Provides on-call coverage. Performs patient triage and determines the urgency of physician's evaluation. Provides symptom management for side effects related to oncologic treatment. STANDARD QUALIFICATIONS Education / Training: Master's Degree in Nursing OR a Masters in Physician Assistant studies Work Experience : 1 to 3 years’ experience as an Advanced Practice Provider in an inpatient setting, preferred.
