Now Hiring – RN On Call Nurse Hours: Friday 10a-10p, Sat and Sun 8a-8p The RN-On Call Nurse is responsible for, but not limited to: Assumes responsibility for a patient visit or on call schedule which includes the assessing, planning, implementing and evaluating phases of the nursing process Provides hospice nursing care based on systematic assessment docusing on pain control and symptom management Initiates communication with attending physicians, other hospice staff members and other agencies as needed to coordinate care and use of resources for the patient/family Maintains regular communication with direct supervisor to review assignments Maintains up to date documentation at point of service so that problems, plans, actions and goals are accurately and clearly stated and charges are reflected as they occur in accordance with agency policy and Medicare regulation Administers medication in accordance with all federal and state laws and the policies of the Hospice along with LTC staff Maintains current patient records and documents in accordance with the policies of Ascend Hospice to ensure that problems, intervention, goals and outcomes are accurately and clearly states and charges are reflected as they occur Assess and collaborates with Medical Director to determine initial and on going eligibility of the hospice patient Completes an initial assessment of patient and family to determine home care needs Can anticipate end stage disease process and related pain and symptom management Regularly re-evaluates patient's nursing needs Uses assessment data to determine care plan needs Initiates appropriate preventive and palliative nursing procedures Counsels the patient and family in meeting end of life needs Performs a comprehensive death note when appropriate Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition Communicates with other members of the IDT to coordinate the care plan Participates in On-call duties as defined by the on-call policy Ensures that arrangements for equipment and other necessary items and services are needed Ascend Hospice - is part of the Ascend family of Healthcare Companies. Full Time LPN delivering quality healthcare at our 75th St Kansas City and Raymore HealthHub locations As a Licensed Practical Nurse, you will utilize advanced clinical expertise to deliver high-quality, evidence-based patient care. Responsibilities include assessing patient needs, administering treatments and medications, collaborating with interdisciplinary teams, and advocating for patients. Your critical thinking and clinical knowledge will help support improved patient outcomes and enhance healthcare delivery. In this role, you will: Administer oral, injectable, intravenous, and topical medications. Evaluate complex patient conditions, monitor pain levels, and document treatment responses. Apply advanced knowledge of vital sign monitoring to identify and record abnormalities in blood pressure, heart rate, respiratory rate, and temperature. Perform routine assessments to monitor patient health and ensure overall well-being. Manage patient access to healthcare and connect individuals with community resources for medication administration, self-care techniques, and disease management. Conduct medication reviews to identify potential side effects and drug interactions. Develop and administer mental health assessments to screen for depression and evaluate mood, behavior, and cognitive function. Accurately document patient medical history—including illnesses, medications, allergies, surgeries, and family history—using electronic health records. Use your excellent communication skills to interact with patients, families and other healthcare professionals To be considered for this important opportunity to care for patients in the community, you must meet the following qualifications: Active and in good standing state LPN license. Successful completion of the NCLEX-PN (National Council Licensure Examination for Practical Nurses). A minimum of one year of experience as a License Practical Nurse (LPN) or Licensed Vocational Nurse (LVN). Knowledge of medical terminology and clinical documentation standards. Experience with phlebotomy, vaccine administration, and point-of-care testing. Excellent communication and organizational skills. Experience working in a clinical setting with diverse cultural dynamics among staff and patients. Proven ability to manage sensitive information ethically and responsibly. Strong problem-solving, critical thinking, and decision-making skills. Proficiency with electronic medical record (EMR) systems; Epic experience preferred. Educational Qualifications: High school diploma or GED Graduate of a state approved practical nursing program. Successful completion of the NCLEX-PN (National Council Licensure Examination for Practical Nurses). Pay Range The typical pay range for this role is: $22.40 - $48.67 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
